Job Summary

We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care and demonstrate the ability to work effectively in various settings, including nursing homes, hospitals, and home care environments. As an RPN, you will play a crucial role in providing high-quality care to patients, assisting with their daily needs, and ensuring their comfort and safety.

Responsibilities

Provide direct patient care, including medication administration and monitoring vital signs. Assist with daily living activities for patients in nursing homes, assisted living facilities, or home care settings. Collaborate with healthcare teams to develop and implement individualized care plans. Conduct assessments and document patient progress using medical terminology and electronic health record systems such as Epic or Cerner. Administer phlebotomy procedures as needed and manage laboratory specimens. Support patients with acute care needs, dementia care, pediatrics, hospice care, pain management, and senior care. Educate patients and their families about health conditions, treatment plans, and preventive measures. Participate in case management activities to ensure continuity of care for patients.

Requirements

Valid Registered Practical Nurse license in the state of practice. Previous experience in nursing homes, hospitals, or acute care settings is preferred. Knowledge of medical terminology and proficiency in medication administration. Experience with electronic health records (Epic or Cerner) is a plus. Strong communication skills and the ability to work collaboratively within a healthcare team. Compassionate approach to patient care with a focus on dignity and respect for all individuals. Experience in specialized areas such as pediatrics, dementia care, hospice care, or pain management is advantageous. Ability to adapt to changing situations and provide high-quality care under pressure.
