We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will have experience in various settings, including nursing homes, hospitals, and home care, and will be skilled in providing high-quality patient care. This role requires a strong understanding of medical terminology, medication administration, and the ability to work collaboratively with other healthcare professionals to ensure optimal patient outcomes.
Duties
Provide direct patient care in accordance with established nursing protocols and standards.
Administer medications and monitor patients for side effects and effectiveness.
Conduct assessments and develop individualized care plans for patients.
Assist with phlebotomy and laboratory tests as required.
Maintain accurate patient records using electronic health record systems such as Epic or Cerner.
Collaborate with multidisciplinary teams to coordinate care for patients in various settings, including acute care, assisted living, and hospice care.
Educate patients and their families about health management, disease prevention, and treatment options.
Provide specialized care for populations including pediatrics, dementia patients, and seniors.
Implement pain management strategies and assist with case management as needed.
Qualifications
Valid Registered Practical Nurse license in the relevant jurisdiction.
Previous experience in nursing homes, hospitals, or home care settings is highly desirable.
Familiarity with medical terminology and procedures related to patient care.
Experience in acute care environments or specialized fields such as pediatrics or dementia care is a plus.
Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare team members.
Knowledge of electronic health record systems (Epic or Cerner) is an advantage.
Ability to perform laboratory tasks and manage medication administration safely.
