We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to adapt to diverse patient needs. The RPN will play a vital role in ensuring the well-being and safety of patients while collaborating with multidisciplinary teams to deliver comprehensive care.
Responsibilities
Administer medications and treatments accurately according to physician orders and established protocols
Monitor patient vital signs and assess health status regularly
Assist with activities of daily living and provide personal care to patients of all ages, including pediatrics and senior care
Document patient information accurately using electronic health record systems such as Epic and Cerner
Perform phlebotomy and laboratory specimen collection as needed
Manage pain effectively through appropriate interventions and medication administration
Coordinate with physicians, therapists, and other healthcare professionals for case management and care planning
Provide dementia, hospice, acute, and chronic illness care tailored to individual patient needs
Educate patients and families about health conditions, medications, and post-discharge care plans
Ensure compliance with healthcare regulations, safety standards, and infection control protocols
Skills
Proven experience in nursing home, hospital, or home care settings with a focus on patient-centered care
Knowledge of medical terminology, medication administration, and clinical procedures
Familiarity with electronic medical records systems such as Epic and Cerner
Strong understanding of dementia care, senior care, hospice services, pediatric nursing, and pain management techniques
Excellent communication skills for effective collaboration with healthcare teams and families
Ability to handle multiple priorities efficiently in fast-paced environments
Experience with case management and laboratory procedures is preferred
Compassionate approach with a commitment to delivering exceptional patient care across diverse populations including children and seniors
