Job Summary

We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care, with experience in various settings such as nursing homes, hospitals, and home care. The RPN will be responsible for providing high-quality nursing care to patients, ensuring their comfort and well-being while adhering to best practices and medical protocols.

Duties

Deliver direct patient care, including monitoring vital signs and assessing patient conditions. Administer medications and treatments as prescribed by physicians. Collaborate with healthcare team members to develop and implement individualized care plans. Provide support for patients with dementia, pediatrics, acute care needs, and hospice care. Conduct phlebotomy procedures and manage laboratory samples as necessary. Maintain accurate medical records using electronic health record systems such as Epic or Cerner. Educate patients and their families on health management and medication administration. Assist in case management duties to coordinate comprehensive patient care. Ensure compliance with medical terminology standards and best practices in pain management.

Qualifications

Valid Registered Practical Nurse license in the relevant jurisdiction. Previous experience in nursing homes, hospitals, or assisted living facilities is preferred. Familiarity with acute care settings and senior care practices is highly desirable. Experience in home care or hospice settings is a plus. Strong understanding of medical terminology and medication administration protocols. Proficiency in electronic health record systems (Epic, Cerner) is advantageous. Excellent communication skills with the ability to work effectively within a multidisciplinary team.
