We are seeking a dedicated and compassionate Registered Nurse (RN) to join our healthcare team. The candidate will be responsible for providing high-quality patient care in the context of Long Term Care Home. As a key member of the healthcare team, you will utilize your clinical skills to assess patient needs, care for health concerns within your scope of practice, and collaborate with other healthcare professionals to ensure optimal patient outcomes.
Responsibilities
Safely prepare and administer routine medications and treatments as prescribed by the physician, adhering to the College of Nurses of Ontario's Medication Administration Standards.
Conduct thorough patient assessments to identify health issues and needs, and develop individualized care plans based on those assessments.
Monitor patients' vital signs and overall health status regularly, providing accurate and timely documentation of all care provided, including any health concerns raised by patients or their families.
Promote patient independence by supporting all aspects of daily living, offering education, and encouraging wellness to help patients reach their maximum capacity.
Collaborate with interdisciplinary teams to coordinate comprehensive patient care and ensure continuity of care.
Provide ongoing support and guidance to unregulated care workers, including direction, training, and assistance with completing their assigned duties.
Complete all required assessments in line with facility protocols, including updating care plans, documenting falls, and delivering timely wound care.
Administer pain management strategies and perform phlebotomy as needed for diagnostic testing.
Educate and support patients and families on health management, promoting informed decision-making and well-being.
Participate in case management activities and maintain accurate electronic health records (e.g., Epic) for all patient interactions.
Perform additional duties and responsibilities as delegated.
Qualifications
Current, uninhibited registration with the College of Nurses of Ontario
Current Basic Cardiac Life Support Certificate (BCLS)
Documented completion Vulnerable Sector Check
Candidates must meet the provincial police check and tuberculosis screening per legislative requirements
