setting. The successful candidate will be responsible for providing quality care to residents, supporting the nursing team, and ensuring the well-being of individuals with chronic illnesses, disabilities, and age-related conditions in a residential care home environment.
Key Responsibilities:
Resident Care:
Provide direct patient care, including administering medications, monitoring vital signs, and assisting with activities of daily living (ADLs) such as bathing, dressing, and feeding.
Clinical Assessments:
Conduct regular health assessments, including monitoring and recording vital signs, lab results, and other medical data. Report any significant changes in the resident's condition to the physician or healthcare team.
Collaboration with Healthcare Team:
Work alongside physicians, registered nurses, personal support workers (PSWs), and other allied health professionals to develop and implement individualized care plans for residents.
Patient Advocacy:
Act as an advocate for residents' rights, ensuring their safety, dignity, and comfort at all times. Ensure residents' families are kept informed of changes in their loved ones' care.
Documentation:
Maintain accurate and up-to-date records of resident care, medications, and progress notes in accordance with regulatory requirements.
Infection Control:
Follow infection control protocols to ensure the safety of all residents and staff. Administer infection prevention practices, including the proper use of PPE.
Emergency Response:
Respond to emergencies as needed, providing immediate first aid, CPR, or other required interventions in accordance with care protocols.
Education & Training:
Provide guidance and support to PSWs, personal care attendants, and other team members as needed. Stay updated on best practices and attend training as required by the organization.
Qualifications:
Licensing/Certification:
Current registration with the College of Nurses of Ontario (CNO) as a
Registered Practical Nurse (RPN)
.
Experience:
Experience in long-term care, geriatric nursing, or home care is an asset.
Skills:
Strong clinical assessment and critical thinking skills.
Ability to work independently and as part of a team.
Excellent communication and interpersonal skills.
Compassionate and patient-focused.
Knowledge of current nursing practices, safety standards, and infection control protocols.
Physical Requirements:
Ability to lift, transfer, and assist residents with mobility as needed.

Long-term care facility with a variety of shifts (days, evenings, nights, weekends).
Physical Demands:
