The Home

: Our accredited, not-for-profit, long-term care home serves a total of 160 residents with dementia whom are no longer able to live safely and independently within our 5 residential home areas.

The Day and Respite Programs

: The McCormick Dementia Services offers the largest day program of its kind in Ontario! Through their commitment to delivering the highest quality programs and services to clients in our day program, overnight respite program and care partner support programs.

Summary:

Overall provision of professional nursing care based on their knowledge of the resident/client, their health and the environment. Overall leadership in the form of the Team Leader by providing guidance and support for the Personal Support Worker. Schedule: 10 pm - 6am

Job Requirements

Current registration with the College of Nurse of Ontario as an RPN. Current CPR Heart Saver Certificate required. Ability to plan care using Resident Assessment Instrument-Minimum Data Set 2.0(RAI-MDS 2.0) Recent related work experience in Geriatrics, Long Term Care preferred. Certification as a "Psychogeriatric Resource" with P.I.E.C.E.S. training, preferred. Gentle Persuasive Approach (GPA) training, preferred. Knowledge of diseases and conditions causing memory impairment in frail elderly populations; experience with memory impaired population. Excellent interpersonal skills and able and willing to work as an effective team member. Demonstrated commitment to relationship-centred care. Demonstrated flexibility and organizational skills. Must be capable of meeting the physical demands of the job and apply effective body mechanics. Demonstrate effective and communication and documentation skills. Ability to follow and give written and verbal instructions. Ability to work in a computerized environment. Demonstrated commitment to relationship-centred care. Must be able to perform all duties utilizing the core Values of McCormick Care Group: Compassion, Accountability, Respect, Excellence, and Collaboration. Shift types include: Days, Evenings, Nights, and Weekends (Two per month)
