We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care and demonstrate proficiency in various nursing practices. This role involves providing high-quality care to patients across different settings, including long-term care, assisted living, and acute care environments. The RPN will be responsible for assessing patient needs, administering medications, and collaborating with healthcare professionals to ensure the best outcomes for patients.
Responsibilities
Conduct comprehensive patient assessments and develop individualized care plans.
Administer medications and treatments as prescribed, including IV infusion and injections.
Monitor vital signs and document patient progress in electronic health records (EHR).
Provide direct patient care in various settings such as nursing homes, hospitals, and outpatient clinics.
Assist with procedures including catheterization, tube feeding, and wound care.
Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.
Educate patients and families on health management, medication administration, and treatment plans.
Ensure compliance with HIPAA regulations and maintain accurate medical documentation.
Participate in infection control measures and maintain a safe environment for patients.
Qualifications
Valid Registered Practical Nurse license.
Experience in critical care, long-term care, or assisted living preferred.
Proficiency in EMR systems such as Epic, Cerner, or Athenahealth is an asset.
Knowledge of ICD-9/ICD-10 coding and medical terminology is beneficial.
Strong skills in patient assessment, triage, and acute pain management.
Previous experience working with individuals with developmental disabilities or dementia is a plus.
Familiarity with geriatrics, pediatrics, or hospice care is advantageous.
Ability to perform basic life support (BLS) and demonstrate knowledge of anatomy and physiology.
