The Centre for Family Medicine's academic Family Health Team is comprised of four sites and is an integral part of the University of Waterloo Integrated Health Building as well as the McMaster Waterloo Regional Medical Campus. MINT Memory Clinics
The Multi-specialty INterprofessional Team (MINT) Memory Clinic is an inter-professional approach to providing early diagnosis and treatment for persons living with memory loss. Team members work with family physicians and allied health professional to provide comprehensive care for patients and family members dealing with dementia and other conditions involving memory loss. Our goal is to improve access to high quality person-centered dementia care for older adults and their caregivers through team-based care provided in primary care.
Position Overview
Reporting to the Elder Health Manager and oversight with the Clinical Director, this is a key clinical position on the team whose role is to provide clinical assessment to individuals experiencing memory loss.
Responsibilities
In partnership with the memory clinic team, the nurse will provide thorough memory assessment. Including: cognitive testing, physical examination, frailty screening.
Prepare and update the patient's EMR with pertinent consultation notes, current bloodwork, previous imaging, cognitive testing and related medical appointments.
Provide bloodwork and imaging requisitions as needed, via a Medical Directive from the physicians
Participate in group conversation and debriefing with memory clinic team.
Will be expected to document and maintain clear records in the patients EMR
Responsible for scanning appropriate documents into patients' chart after completed visits
Should be able to communicate effectively and professionally with all patients, care partners and other staff to provide a high-quality care experience.
Assisting all team members when needed.
Requirements
RPN registration with the College of Nurses of Ontario and in good standing
Demonstrates competence within the full scope of practice of an RPN
Knowledge of College of Nurses of Ontario (CNO) Standards of Practice
Experience with electronic medical records
Current CPR certification
Experience working in inter-disciplinary teams and liaising with clinicians, patients, families and other care givers
Strong analytical and problem-solving skills
Solid organizational skills including attention to detail and multitasking skills
Ability to work well with multiple teams across multiple locations
Ability to work with ambiguity and minimal supervision
Excellent written and verbal communication skills
Experience working with individuals with memory loss is an asset
Compassionate verbal communication skills
