Unit Profile:



The Multi-Care Kidney Clinic (MCKC) at St. Joseph's Healthcare Hamilton is a multi-disciplinary clinic that provides care, support, and education to patients diagnosed with end stage renal disease. This busy clinic environment will see the Registered Practical Nurse working as a member of the interdisciplinary team in supporting a large and diverse clinic population at the Hamilton and Brantford clinic locations. In addition to supporting the MCKC, the Registered Practical Nurse supports the General Nephrology Clinics, including hypertension, pregnancy, onconephrology, glomerulonephritis, and transitional clinics.



Position Summary:



We are seeking a highly motivated Registered Practical Nurse to join our team. The Registered Practical Nurse is an integral part of the team responsible for overseeing the daily activities of the clinic by planning and delivering all nursing-related care within the clinic. The Registered Practical Nurse provides basic assessment and treatments in addition to preventative, educational and/or supportive care. The RPN assesses and supports patient and/or caregivers understanding of health condition and available treatment options. The Registered Practical Nurse promotes collaborative team practice amongst all disciplines and acts as a resource for all staff members, communicates effectively with members of the health care team on the unit and other patient care areas. The Registered Practical Nurse adapts to changing workloads while collaborating with colleagues regarding adjusting priorities. The Registered Practical Nurse facilitates the planning and coordinating of nursing care interventions with the delivery of interdisciplinary care.



Qualifications:



Current registration with the College of Nurse of Ontario required

Successful completion of a provincially recognized educational program for Registered Practical Nurses required

Minimum 1 year of previous experience in renal nursing required

Current BCLS/HCP certification required

Demonstrated Knowledge, Skills, and Abilities

Demonstrated ability to function to the full scope of nursing practice per the College of Nurse of Ontario.

Demonstrated knowledge, sensitivity and expertise in the care of seniors and their families.

Demonstrated ability to communicate effectively (written and verbal) with patients/clients, families and other members of the health care team.

Demonstrated ability to practice independently in conjunction with the multi-disciplinary team.

Demonstrated ability to set priorities and organize work in response to client/patient needs.

Basic skill level with computers (e.g., Microsoft Office [Word, Outlook, etc.], internet browser searches, data entry and patient information systems).

Demonstrated commitment to ongoing learning, professional development and continuous quality improvement.

Excellent attendance record is required.

Wage Rate:



$35.12... $36.64... $38.16

(plus 14% in lieu of benefits)
