To assist with the provision of therapeutic social/recreational activities, Activities of Daily Living (ADLs), medication administration, day and overnight monitoring and behavior interventions for Adult Day Program and Overnight Stay Program participants (which includes frail elderly, dementia, mental health and acquired brain injury population).
Specific Responsibilities:
Document treatment protocols and medications administered, unusual incidents, near misses, advice given to client or caregiver and any other pertinent information on client chart
Assist in maintaining a safe physical and psycho-social environment.
Assist the Overnight Stay Team and Clinical Resource Worker with the completion of day program client profiles and care plans
Assist with the implementation and organization of client Instrumental Activities of Daily living; Assist clients with activities of daily living.
Participate in meal preparation and in recreational activities
Comply with St. Joseph's Health Centre's Privacy policy based on PHIPA principles.
"Trouble shoot" for staff, participants, families, and physicians.
Promote and support ongoing staff professional development.
Provide nursing care in accordance with administrative policies, established standards outlined in the "Standards of Nursing Practice" from the College of Nurses of Ontario ? Provide support and assistance during staff absences.
Administer medications as prescribed by participant's physician and requested by participant's family and maintain an accurate record in MAR.
Assess participant's ability to ambulate and perform lifts and transfers, assist participant with ambulation as necessary.
Provide appropriate interventions with participants exhibiting responsive behaviours, communicate and document outcomes.
Respond appropriately to emergency situations and provide direction to other team members.
Qualifications:
Current registration with the Ontario College of Nurses.
Current CPR certificate
Registration with or completion of a Geriatric Certificate Program (preferred through the Regional Geriatric Program)-UFIRST or PIECES Certification
Advanced knowledge of diseases and conditions causing memory impairment
Experience with community based cognitively impaired, frail elderly population
Experience in administering medications and monitoring client response to medications
Experience with recreational program planning and delivery
Excellent documentation, organizational, interpersonal and communication skills.
Knowledge of pertinent community support services preferred.
Ability and willingness to work as an effective team member.
Ability to apply effective body mechanics and meet physical demands of the position.
Knowledge of and/or willingness to pursue learning in the InterRAI CHA and PIECES assessment tools
Completion of Pharmacology certificate preferred
Food Handling Certificate preferred
