Job Description

Requirements:
- Valid LPN license
- Minimum 3 years of experience in acute care settings, preferably with experience in pediatric care.
- Proficiency in using electronic medical record systems, such as Cerner.
- Strong knowledge of medication administration practices and protocols.
- Experience in hospice care and providing end-of-life support is preferred.
- Ability to effectively communicate with patients, families, and healthcare professionals.
- Compassionate and empathetic approach to patient care.
- Excellent critical thinking and problem-solving skills.
- Ability to work well in a fast-paced environment and prioritize tasks efficiently.

Responsibilities:
- Provide direct patient care in various healthcare settings, including medical offices, assisted living facilities, and hospice care.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' conditions and report any changes to the healthcare team.
- Perform phlebotomy and collect specimens for laboratory testing.
- Assist with pain management and provide comfort measures to patients.
- Provide specialized care for patients with dementia or other cognitive impairments.
- Collaborate with interdisciplinary teams to develop and implement care plans.
- Educate patients and their families on healthcare procedures and self-care techniques.
