Overview

We are seeking a compassionate and skilled Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will have a strong background in patient care and be dedicated to providing high-quality nursing services in various settings, including nursing homes, hospitals, and home care environments. This role requires a commitment to the well-being of patients, with a focus on delivering personalized care and support.

Responsibilities

Provide direct patient care, including administering medications and treatments as prescribed. Monitor and assess patient conditions, documenting changes in medical records using Point Click Care Assist with daily living activities for patients in an assisted living facilities. Collaborate with healthcare team to develop and implement individualized care plans. Collect and manage laboratory specimens as needed. Educate patients and their families about health conditions and behaviour management. Maintain accurate medical terminology and documentation standards throughout patient interactions. Support pain management strategies for patients experiencing discomfort or chronic pain. Provide specialized care for patients with dementia, or those requiring hospice services. Engage in case management activities to ensure comprehensive patient support.

Qualifications

Valid Registered Practical Nurse licensed by the CNO Previous experience in nursing homes, hospitals, acute care settings is preferred. Familiarity with medical terminology and strong documentation skills are essential. Experience with electronic health record systems such as Point click care Strong interpersonal skills to effectively communicate with patients, families, and healthcare teams. Ability to provide compassionate care to diverse populations including children and seniors.
