We welcome all interested persons who self-identify as Indigenous, Black/African Nova Scotians, Persons of Colour, Immigrants/Newcomers, Persons with Disabilities, 2SLGBTQIA+ to apply to support our goal for our workforce to be representative of the patients, families, and communities that we care for at all job levels.The OpportunityReporting to the Clinical Manager, Community Mental Health and Addictions, the RN works closely with the other members of the Specific Care Clinic staff to provide nursing care that is integrated and compatible with the clinic\'s patient-focused goals and objectives.The RN is responsible and accountable for the delivery of patient and family centered patient care consistent with the philosophy, objectives, vision and mission of the IWK Health Centre. The Staff Nurse\'s focus will primarily involve an ambulatory care population, but the provision of care in inpatient/acute care settings may also be required. The RN role includes case coordination, transition support, telephone facilitated care, psycho-education, medication management and monitoring, assessment & intervention. The nurse\'s role will encompass physical/mental health assessment at different points of care including referral, intake, treatment and consultation. The nurse will liaise with and engage in capacity building through the provision of education and consultation services to others across the continuum of care. The RN will also liaise with family doctors to bridge communication between different care areas and treatment teams. Collaboration with interdisciplinary care team members involved in patient care essential.The RN practices according to the Canadian Nurses Association Code of Ethics for Registered Nurses, the College of Registered Nurses of Nova Scotia Standards of Nursing Practice, specialty standards as applicable and the policies and procedures of the IWK.Hours of Work7.5 hour shifts, Monday to Friday, 0830-1630, with some evenings.
