Your Opportunity:

As a self-directed team member with Community Care in the Edmonton zone, the Registered Nurse demonstrates strong team player skills and has responsibilities, including; assessment of needs, delivering appropriate therapeutic nursing services within professional practice standards, coordinating resources, establishing goals for client delivery, and evaluating outcomes. In addition, this position involves case management of clients as well as the direct provision of client care that meets established clinical standards, and is evidence based and client centered. The Registered Nurse acts as a consultant on the team providing clinical expertise and direction to other team members regarding their cases. Provides case management through nursing assessment and interventions to clients. This position is integral to the Short Term Intervention Team (STIT), which provides triage and clinic support, as well as community visits that require specialized skills in cardiac, IV therapy and wound care. Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:

Proven communication and assessment skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast paced environment. Computer Proficiency and experience. Strong professional judgment, initiative, and effective time management skills are necessary. Ability to work in different environments within the community (single family homes, lodges, walk up apartments, etc.) performing nursing tasks as well as lift and carry professional bags/equipment weighing up to approximately 25-30 lbs. Preferred Qualifications:

Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in the community sector will be considered. Five (5) years current nursing practice with at least one year in community based setting. Previous Community Care and/or Home Care experience preferred. Experience with Meditech and the RAI Home Care assessment tool is preferred.
