Your Opportunity:

Working collaboratively as part of the Short Term Intervention Team (STIT), the Registered Nurse (RN) STIT Case Manager is involved in the case management of clients, including ongoing assessment, determination of unmet need, and the provision of direct nursing care requiring specialized skills in cardiac, IV therapy and wound care. Case Managers work within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating and building relationships. The RN STIT Case Manager ensures that client care is evidence based and client-centered by establishing goals for service delivery and evaluating outcomes. In addition, this position provides triage and clinic support.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.17/hour - $42.96/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:

Proven communication and assessment skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast-paced environment. Strong professional judgment, initiative, and effective time management skills are necessary. Ability to work in different environments within the community (single family homes, lodges, walk up apartments, etc.) performing nursing tasks as well as lift and carry professional bags/equipment weighing up to approximately 25-30 lbs. Preferred Qualifications:

Baccalaureate Degree in Nursing preferred; however, skills, and experience relevant to nursing practice in community sector will be considered. Two years current nursing practice with one year in a community-based setting is preferred. Experience with cardiac, IV therapy and wound care is preferred.
