Job Description
POSITION:
Care CoordinatorDUTIES AND RESPONSIBILITIES
Client Care:
Schedule and conduct intake visits for new clients
Complete initial assessment of each client and follow-up assessments as required
Create a plan of care for each client and update as required:
o Type of visit (PSW, Caregiver.)
o Frequency and length of visits
o Communicate with Team Assistants for best fit
o Services provided at each visit
o Payment options
Reviews progress notes made by field staff
Wellness Checks
Delegate skills to field staff
Address concerns brought forward by family members/guardians
Address concerns from filed staff
Job Type: Casual
Pay: From $30.00 per hour
