community nursing
and a
minimum of two (2) years of wound care experience

The RN must be in
good standing with the provincial College of Nurses
and possess the ability to provide comprehensive, evidence-based care across multiple healthcare settings, including hospitals, home care, and assisted living environments. This position plays a vital role in delivering high-quality nursing services and fostering positive health outcomes for patients.
Key Responsibilities
Conduct
comprehensive patient assessments
and develop individualized care plans.
Administer medications safely and monitor patient responses to prescribed treatments.
Perform
wound assessments, dressing changes, and advanced wound management
in accordance with clinical best practices.
Deliver
community-based nursing care
, supporting patients in their homes and residential care facilities.
Maintain accurate, timely, and complete clinical documentation in accordance with organizational and regulatory standards.
Collaborate effectively with interdisciplinary healthcare teams to ensure continuity of care, including
case management
and
pain management
strategies.
Provide
education, guidance, and emotional support
to patients and families regarding treatment plans, health conditions, and self-care practices.
Participate in
senior and chronic disease care programs
, including dementia care and rehabilitation initiatives.
Adhere to all
infection prevention, safety, and quality assurance protocols
, ensuring a safe and therapeutic care environment.
Qualifications and Requirements
Current Registered Nurse (RN) license
in the province of practice, in
good standing with the College of Nurses
.
Minimum of two (2) years of wound care experience
in community or clinical settings.
Demonstrated experience in
community nursing
, including home care, long-term care, or assisted living.
Previous experience in hospital or residential care environments is considered an asset.
Strong clinical assessment, critical thinking, and organizational skills.
Excellent interpersonal and communication abilities to collaborate effectively with patients, families, and healthcare professionals.
