, as the patient requires specialized dialysis support. The right candidate will be confident in managing dialysis procedures in a home setting and monitoring for any complications.

Schedule:
Tuesdays, Thursdays, and Saturdays from 11:00 AM to 3:00 PM
Responsibilities
Dressing Change for CVAC or PICC
Administer medications and treatments as prescribed by physicians.
Conduct patient assessments and monitor vital signs.
Assist with daily living activities and provide personal care to patients.
Document patient care accurately in electronic health records (EHR) systems.
Collaborate with healthcare professionals to develop and implement patient care plans.
Provide education to patients and families regarding health management and disease prevention.
Maintain a clean and safe environment for patients.
Participate in quality improvement initiatives and adhere to infection control protocols.
Support patients with chronic conditions, including Alzheimer's care, dementia care, and memory care.
Experience
Candidates should have experience or knowledge in the following areas:
Hemodialysis
Epic or Cerner EMR systems for efficient medical documentation.
Critical care experience, including ICU or PICU settings.
Medical office experience with familiarity in outpatient procedures.
Long-term care experience in nursing homes or assisted living facilities.
IV infusion skills, including IV insertion and medication administration.
Experience working with individuals with developmental disabilities or in hospice care.
Proficiency in basic life support (BLS) techniques and phlebotomy skills.
Familiarity with pain management strategies, acute pain management, and geriatrics.
