We're looking for just the right person to join our team as Health and Wellness Coach - Lake Country.

You will be committed to Making Lives Better for people after brain injury and stroke, collaborating as part of an integrated, multi-professional Coaching Resource Team in the development of individualized Life Redesign Plans for the people we support. You will manage the health and wellness aspects of a person's experience at CONNECT, participate in the mentoring and development of staff, contribute to our continuous learning program, and help us in maintaining CONNECT's accreditation status with Accreditation Canada.

About the Role

Health & Wellness Coaches are RNs who blend clinical knowledge with coaching to support individuals living in real-world, community-based settings. This isn't traditional nursing--it's about building capacity, encouraging autonomy, and helping people manage their health and wellness as part of a meaningful life redesign.

Health and Wellness Coaches work as part of an integrated team with coaches from other disciplines and Life Redesign Coaches (support staff), who support people in their homes. A Health and Wellness Coach serves as a nurse, coach, mentor and teacher for the people we support, families, and co-workers, informs the development of appropriate support strategies for individuals and staff, providing nursing skills as needed for the individual's health care needs and lifestyle choices.

The Health and Wellness Coach contributes to CONNECT's orientation and continuous learning, and helps build on standards for excellence, and commitment to continuous quality improvement.

A Health and Wellness Coach reports directly to the site Leader of Culture, People and Services.

What You'll Do

Able to complete nursing assessments to identify a person's health care needs and how to best support those needs through internal and/or external services. Able to delegate tasks to Life Redesign Coaches while providing continuous coaching, education, guidance and evaluation. Able to provide direction to Life Redesign Coaches regarding urgent health needs both during and after hours (as part of our emergency on-call team) Able to follow and implement physician directions and orders. Able to contribute to, and occasionally lead conferences with the people we support, their families, funding partners, and physicians. Able to communicate effectively with the people we support, families, Life Redesign Coaches, interdisciplinary team, physicians, and funding partners. Able to create, and contribute to, an atmosphere of safe, quality, person- and family-centered care. Able to follow and implement Public Health infection prevention and control measures. Able to complete professional reports and assessments (InterRAI, WSBC reports, Braden scales, seizure protocols, serious incident reports etc.). Understand a person's story and the factors that influence their priorities, choices and life goals; bring forward your thoughts, ideas and recommendations for consideration and inclusion as measurable, actionable steps to success in their individual life redesign plan. Make best use of emergent opportunities for the individual's real-life learning.

What We're Looking For

Registered Nurse (RN) in good standing with BCCNM. Experience supporting people with acquired brain injury, stroke, or complex needs. Strong coaching, mentoring, communication and relational skills. Flexible, adaptable, and collaborative in dynamic environments. Comfortable working outside traditional clinical models. A commitment to CONNECT's values, including joy, creativity, and person-led practice.

This is a presence-based role that involves working closely with individuals, families, and team members across CONNECT's community homes. Health & Wellness Coaches have some flexibility in managing their schedules and may complete certain tasks remotely with prior approval. There is an on-call component to this role. You will be part of a collaborative, strengths-based team helping people live full, self-directed lives.

CONNECT Lake Country, located in Lake Country our building comprises six individual homes. Each home includes six bedrooms, to support up to 42 people at any one time. You will have the important job of representing CONNECT and building strong community relationships as a good neighbour, partner and friend, so we walk this important journey together.
