Detailed Overview:
Reporting to the Manager or designate, the Registered Nurse (RN) LTC Float /Medical provides nursing care to residents/patients consistent with current Standards of Professional Practice and the Nursing Philosophy of Fraser Health. The Registered Nurse plans, organizes and implements nursing care for residents/patients receiving medical care.
Responsibilities: Provides direct nursing care to residents/patients receiving medical care based on nursing knowledge and in conformance with established standards and Fraser Health policies by performing such duties as assessing resident/patient care needs, identifying problems, planning, implementing, evaluating and documenting nursing care. Modifies care as needs change, initiates immediate treatment if required and communicates changes to the appropriate personnel in a timely manner.
Collaborates with physicians and other health care providers in planning and evaluating the care provided.
Documents relevant observations, assessments and interventions; including resident/patient and family teaching and evaluation of care using appropriate technological tools (for example, InterRai).
Acts as an advocate to protect and promote residents'/patients' rights to autonomy, respect, privacy, dignity, diversity and access to information in accordance with standards of professional practice and philosophy of Residential Care/Acquired Brain Injury Services.
Plans and provides teaching to residents/patients and their families by identifying health-related learning needs, providing related teaching and/or referring to the appropriate resources (e.g. respiratory therapy, home care, cardiac rehab programs, diabetes education center etc.).
Liaises with Diagnostic Imaging, Laboratory and Respiratory Services to coordinate tests and procedures for residents/patients receiving medical care.
Initiates discharge planning for residents/patients on admission, intervenes as appropriate and refers to other disciplines (e.g. social work).
Communicates, participates and consults with members of the health care team about resident/patient care and participates in multidisciplinary conferences.
Participates in the collegial sharing of information among staff; identifies self-learning needs and attends educational programs to maintain and enhance clinical competency.
Participates in the orientation and ongoing education of nursing staff and students by providing information and acting as a preceptor, as appropriate.
Participates in quality improvement activities by maintaining information relevant to the management of the assigned resident/patient group and providing input to the RCC/PCC or designate regarding the development of indicators, standards, practices and procedures; prepares reports as required; participates in related research activities by identifying resident/patient care and/or work environment issues and collecting data.
Maintains a safe and therapeutic environment for residents/patients, visitors and staff by adhering to established FraserHealth standards related to infection control, fire and other emergency practices and procedures.
Performs other related duties as assigned.
Qualifications:
Education and Experience
Graduation from an approved program in Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). One (1) year's recent, related acute care and gerontological nursing experience or an equivalent combination of education and experience. Current Level 1 First Aid certificate.
