PURPOSE OF THE ROLE

:


As a member of the Palliative Community Outreach Team (PCOT), the Registered Nurse - Outreach, collaborates with Outreach Physicians, the Clinical Navigator, Psycho-social practitioners, and the Hospice Director to provide high-quality palliative care within the community. Aligned with the Mission, Vision, Values, and Strategic Priorities of the Hospice, the Registered Nurse - Outreach, strives to deliver excellence in hospice palliative care, working within a shared care framework and in partnership with home and community care.


Reporting to the Director, Stedman Community Hospice, the Registered Nurse - Outreach will achieving excellence in hospice palliative care by focusing on relieving patient and family suffering through assessment of physical, psycho-social and spiritual needs, providing symptom management and educating families about the dying and end of life processes.

KEY ACCOUNTABILITIES & SUCCESS

CRITERIA:



In collaboration with the Clinical Navigator, PCOT nurses and Physicians, assesses and triages new referrals to ensure timely response for initial assessments. Completes regular check ins with patients and caregivers to determine priority of caseload needs and schedules necessary home visits for palliative emergencies, initial assessments along follow-up visits. Provides regular and ongoing health teaching to both patients, family members and caregivers to improve understanding of illness trajectory, assist in informed decision making and reduce fear/confusion often associated with navigating the medical system. Utilizes a patient and family-centered approach to care to ensure that needs, wishes and preferences for care are openly communicated and documented. Works closely with Hospice physicians to assess and triage complex symptom management needs and ensures establish plans of care are communicated to agency providers and home care Case Management. Uses specialist skills and knowledge to manage palliative caseload including the support to families/carers with complex needs associated with living with a life-limiting illness. Works autonomously and functions as an expert in palliative care, providing strategic and clinical leadership to influence the delivery of local palliative care throughout the community. Provides ongoing mentorship in the form of phone support to community agency nurses to help empower them in the delivery of high-quality palliative and end of life care for our mutual patients. Collaborates with Hospice staff, primary healthcare teams, specialities clinics, community agencies and volunteers to promote a fully integrated team working with partners and providers. Influence the continued development of palliative care service delivery through the knowledge and implementation of national and provincial recommendations such as the Gold Standards Framework and the Palliative Care Health Services Delivery Framework (OPCN). Collaborates with the Medical Director and Hospice Director as needed, to review sentinel events associated with patient care. + Sentinel events include: all clinical incidents that result in significant injury or harm to a patient; verbal or written threat of litigation on behalf of a patient; receipt of Notice of Action or a Statement of Claim by or on behalf of a patient alleging injury as a result of medical malpractice or medical treatment. Ensures the quality of healthcare provided to patients and clients meets the highest standards as per the College of Nurses of Ontario (CNO) and is in accordance with the Regulated Health Professionals Act. Ensures compliance with relevant provincial and federal legislation governing care provided through the outreach services. Participates in quality assurance processes at the Hospice. Serves as a primary steward in protecting and upholding patients' right to privacy and confidentiality in addition to holding team members to the same standard. Acts as an advocate to protect and promote the patient and family rights to autonomy, respect, privacy, dignity and access to information. Participates in weekday morning huddles with team to discuss patient and family care needs. On weekends and after hours during the week, receives and manages calls for the Outreach Program forwarded by in-house nurses as needed. Completes all clinical documentation in accordance with CNO standards using Hospice EMR (i.e. InfoAnywhere).

QUALIFICATIONS & SKILLS:



Graduate of a recognized nursing program. Registered Nurse (RN) with current registration in good standing with the College of Nurses of Ontario (CNO). Extensive experience in palliative care, preferably in a community or hospice setting. CHPCN(c) or CAPC certified. Ability to work autonomously and collaboratively within an interdisciplinary team. Excellent communication and interpersonal skills, with the ability to advocate for patient and family needs. Proficiency in using electronic medical records (EMR) and other relevant healthcare documentation systems. Effective communication, problem solving and conflict management skills.
