About Community Healthcaring Kitchener-Waterloo (The Health Centre)



The Health Centre provides a broad range of services aimed at improving the health of

individuals and the wider community. Our services include medical and dental care, health

promotion, illness prevention and outreach programming. Our expertise and resources are

focused on working with those who experience health inequities and barriers to accessing health

services. Collaborating as an interdisciplinary team, we provide services and supports that

address the social determinants of health including factors and barriers such as poverty,

inadequate housing, food insecurity, substance use, language and culture, as well as stigma and

social exclusion.



The Health Centre works with the most complex client population served by primary health care

professionals within Waterloo Region. Our highly skilled and diverse team works from a

strength-based approach with individual clients as well as the broader community. We

collaborate with many partner agencies and organizations within Waterloo Region. Our team

consists of Family Physicians, Nurse Practitioners, Registered Nurses, Social Workers,

Dietitians, Chiropodists, Pharmacists, Psychiatrists, Health Promoters, Community Health

Workers, Administrative Staff and Volunteers.

The Health Centre is currently recruiting for the following position:



REGISTERED NURSE or REGISTERED PRACTICAL NURSE



Full Time Permanent 1.0 FTE





Position Summary



Part of the Refugee Health team. Provide direct primary care within their full scope of practice

and participates in the development, implementation, monitoring and evaluation of services for

individuals, families, and the community. The nurse works collaboratively as a member of the

interdisciplinary team and from social determinants of health and assets-based approach.



Specific Responsibilities



1. Complete brief history of clients, anticipate and ensure equipment for client visit is

set up in room.

2. Triage client's needs, either by telephone or in person.

3. Screen and assess clients with minor episodic illnesses according to established

protocols.

4. Perform basic nursing skills (e.g., dressing of wounds, injections, suture /clip removal

and assisting with other procedures as requested).

5. Provides Health education/counselling and other clinical activities in accordance with

the College of Nurses of Ontario standards of practice for Registered Nurses or

Registered Practical Nurse and all relevant provincial legislation and regulations.

6. Perform necessary clinical functions to facilitate delivery of comprehensive primary

care and patient flow (e.g., assist providers with prescription ordering; clean

equipment).

7. Perform nonclinical duties such as the ordering and distribution of supplies.

8. Maintain accurate health records and up to date clinical notes in accordance with

College of Nurses Standards and Community Healthcaring KW policies.

9. Participate in case conferences to ensure co-ordination of services to client.

10. Assist with the planning, implementation and evaluation of other health

promotion/disease prevention programs and services for individuals and client

populations as determined by CHCKW.

11. Ensure all exam rooms are stocked sufficiently and consistently.

12. Order medical supplies and maintain inventory controls.

13. Maintain contact with pharmaceutical company representatives and maintain a

current inventory of pharmaceutical samples.

14. Organize printed resources such as client education materials, diagnostic and other

requisitions.

15. Participate in the development, implementation, monitoring and evaluation of health

promotion/disease prevention programs for individuals and client populations as

determined by the Health Centre.

16. Develop and monitor operating budgets for assigned projects/programs and ensure

expenditures remain within budget.

17. As a Student Supervisor, provide orientation, ongoing support, coaching and

monitoring to meet the requirements of the Health Centre and of the educational

institution.

18. As a Volunteer Supervisor, provide support, training, monitoring and on-going

communication as needed for the volunteer to successfully fulfill their position

description.

19. Maintain a valid driver's license, access to a reliable personal vehicle and adequate

insurance for work-related travel in accordance with the Health Centre's policies.

20. Work during both regular and extended hours (evenings/weekends) and in Outreach

locations within Kitchener as determined by the Health Centre. A minimum of one

evening per week will be required.

21. Participate in team and staff meetings and other meetings and committee work as

appropriate to support both service delivery and organizational goals.

22. Work from a Social Determinants of Health perspective to improve health outcomes

for individual clients and/or for the development of community health and well-being,

and to reduce health inequities.

23. Work individually and collaboratively in the development and implementation of

quality improvement initiatives and activities.

24. Work in a manner that preserves privacy and confidentiality.

25. Work in a manner that ensures client safety and minimizes risk to clients, volunteers

and the Health Centre.

26. Work in a manner consistent with the Health Centre's Vision, Mission and Values.

27. Work in a manner consistent with the Health Centre's policy and commitment to

Diversity, Equity and Inclusion in all processes and practices.

28. Work in a manner that demonstrates self-reflection and personal accountability for

work performance.

29. Contribute to the Health Centre's practices of hiring, orienting and training of staff.

30. Collect, analyze and report on data and relevant information as required.

31. Participate in research as determined by the Health Centre.

32. Adhere to all Health Centre policies.

33. Adhere to appropriate infection control practices including those related to Covid -19.

34. Maintain status of fully vaccinated for Covid-19, including boosters as they become

available or exemption documentation if appropriate.

35. Perform any other duties as assigned by your supervisor.



Accountability



The position is accountable to the Manager, Primary Care.



Qualifications



1. Current registration as a Registered Nurse or Registered Practical Nurse in good standing

with the College of Nurses of Ontario.

2. A minimum of two years employment experience, including in a primary care or

community setting. A combination of primary care, community and acute care

experience will be considered.

3. Knowledge of available resources accessible to clients (e.g. food banks, local shelters,

mental services)

4. Experience working with Refugee Population is considered an asset.

5. Current and ongoing CPR certification.

6. Demonstrated ability to communicate effectively in English, both written and verbally.

7. Second language is an asset.

8. Competency in MS Office Suite programs

9. Proficiency in the use of Microsoft Office Suite. Experience with electronic health

records and other databases is an asset.

10. Ability to work independently as well as an effective member of an interdisciplinary

team.

11. Ability to work collaboratively.

12. Ability to maintain strict confidentiality.

13. Experience working sensitively with culturally and economically diverse populations.

14. Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre.

15. 2-Factor Authentication for Outlook is App based (Microsoft Authenticator Application)

and used to ensure the security of our online accounts. 2-FA requires the use of a

personal mobile device with a recent operating system.



Compensation



Registered Nurse



$63,488.69 - $76,493.02 per annum



Registered Practical Nurse



$52,148.97 - $62,830.00 per annum



Hours:



37.5 hours per week

Monday, Tuesday, Thursday 9:00am- 5:00pm

Friday 8:00am - 4:00pm

Wednesday: 9:00am- 5:00pm or 12:00pm- 8:00pm
