Your Opportunity:The Rockyview Day Hospital is an alternative to inpatient admission within Addiction and Mental Health acute care services. This four-week program operates on a stabilization model and supports patients in enhancing coping skills. The Intake Coordinator is a key member of Day Hospital, managing referrals and screening patients referred to the program in order to assess suitability. In addition, the Intake Coordinator will assist with individual and group facilitation as needed. The Intake Coordinator will be responsible for the intake, clinical interviews and screening of clients with concurrent addiction and mental health disorders. You will liaise with a variety of internal and external stakeholders and work collaboratively with the multidisciplinary team in the delivery of care for the needs of individuals, families and/or communities. In addition, you will be responsible for timely documentation of clinical activities, clinical data entry and regular participation in clinical consultation, team education and ongoing program planning, development and evaluation. You may be required to provide supervision to practicum students. A demonstrated commitment to collaborative practice to enhance client health outcomes; ability to set realistic expectations and boundaries in the therapeutic relationship; strong interpersonal and communication (written and verbal) skills; ability to effectively manage change; ability to work autonomously and collaboratively in a multidisciplinary team; crisis management; and knowledge of psychotherapeutic modalities are requirements of this role.Description:As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.
