Job Summary
The Registered Nurse IIC (Care Facilitator) provides nursing leadership within the Family Care Team Clinic and collaborates with the Manager to facilitate the overall operation of the clinical area. The Care Facilitator engages in quality initiatives such as audits, occurrence reports, workplace measurement statistics, research, and evidenced-informed practice. The principles of the Family Care Team align with the patient health home model of care, using a collaborative health care team approach from various health disciplines in the provision of primary health care services.

The Care Facilitator provides leadership and ongoing support and clinical expertise to nurses, physicians, and members of the interdisciplinary team; and offers advice and guidance in relation to trends/issues affecting the delivery of services. They coordinate strategic programming for the clinic to ensure the clinic services/processes are running smoothly and efficiently. As a leader within the Family Care Team Clinic, the Care Facilitator provides guidance, mentoring and coaching to professional and non-professional staff; identifies education and orientation needs of staff to ensure educational compliance; identifies quality and staffing issues and outlines action plans for resolution; and develops new policy/procedures/protocols or makes changes to existing ones. Other related duties may be required as needed.

Flexible hours are required (combination of days, evenings, and weekends required).

The Registered Nurse IIC reports to the Regional Primary Health Care Program Manager responsible for the service.

Occasionally moves/lifts objects between 10 - 30 lbs., pushing and files, charts and equipment.
Constantly sits to perform work on a computer to do research, write policies/procedures/manuals, prepare presentations, and read and prepare documents.
Regularly consults/interacts with other departments, units, and hospitals to coordinate timely patient/client/resident flow and to communicate regarding division/unit issues.
Regularly stands or walks to perform audits, give presentations, respond to patient/client/resident complaints and collaborate with the health care team to resolve issues.
Regularly performs fine finger/precision work on the computer or with machines that require accurate control and steadiness.
Visual concentration is required to view and report on documents, assess workplace situations, and/or to monitor the workplace regarding health, safety and risk management issues.

Job Qualifications
Graduation from an approved/recognized post-secondary nursing education program with a Baccalaureate degree in Nursing or equivalent education or training is required.
Current practicing licensure with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) is required.
Minimum of five years of experience within the last seven years working in an autonomous/independent practice area is required, including at least two years of experience working in Primary Health Care.
Current BLS Certification is required.
Knowledge of continuous quality improvement concepts.
Demonstrated competency in organizational, leadership, interpersonal, team-building, communication, teaching and decision-making skills.
Demonstrated initiative and ability to work independently.
Demonstrated progressive leadership.
Evidence of active involvement or willingness to participate in continuing professional education development.
Membership in professional organizations/special interest groups is an asset.
A valid Newfoundland and Labrador Class 5 Driver\'s License and the use of a private vehicle are conditions of employment.
A satisfactory record of work performance and attendance is required.
