Bridgepoint Family Health Team Site (430 Broadview Ave)
The Bridgepoint Family Health Team (BFHT) provides primary health care, including chronic disease management, health promotion and disease prevention, to a rostered population of patients from the Riverdale community and across the GTA. The BFHT employs an inter-professional team of health care providers who work in a collaborative model of care. Our interdisciplinary team includes physicians, nurse practitioners, registered nurses, social workers, dietitians, medical assistants, pharmacists, and medical receptionists.
We are currently seeking a Registered Nurse to fill a permanent, full-time position. Working at the full nursing scope of practice and as part of an inter-professional team, the registered nurse will provide primary care services to patients of the Bridgepoint Family Health Team.
DUTIES & RESPONSIBILITIES
Use knowledge, skill and judgment to assess patient's health needs, discuss test results and respond to questions pertaining to health promotion and disease prevention over the phone and in-office.
Provide episodic care, under medical directives.
Provide full scope preventative health care for well-women (including pap smears) and babies under medical directives.
Provide health coaching to meet patient needs, including smoking cessation, healthy eating, increasing physical activity etc.
Previous training in health promotion, and self management, is considered an asset.
Maintain accurate, concise and confidential EMR documentation.
Monitor publicly-funded immunization stock and order immunizations weekly as needed.
Actively participate in program planning, quality improvement projects and other team and staff meetings.
Mentoring/supporting the work and development of Medical Assistants
QUALIFICATIONS:
Current unrestricted registration with the College of Nurses of Ontario
Demonstrated ability to work independently and collaboratively as a member of an interdisciplinary team
Experience with chronic disease management and patient self-care strategies
Primary care experience
Strong patient assessment and triage skills
Experience participating in quality improvement initiatives
Experience in mental health, preferred
Experience with electronic medical records, preferred
SKILL & QUALIFICATIONS CONSIDERED ASSETS:
Solid knowledge of vaccines, including Ontario's Publicly Funded Immunization schedule and primary series childhood vaccines
Experience carrying out Well Woman exams and pap tests; training through McMaster University's Well Woman Workshop is considered an asset
Comfortable using Windows platform and Word processing programs
Familiarity with an EMR essential; experience with Telus PS Suite an asset
Up to date CPR training an asset
Ability to conduct Internet searches in order to access public health guidelines, patient resources, and other topics as needed
Interest in participating in team committees and quality improvement initiatives at the clinic
Interest in participating with program planning such as seasonal flu shot clinics and back-to-school vaccine clinics
