The Registered Nurse (RN) will play an integral role in the care of the patients of the Aurora-Newmarket Family Health Team (ANFHT). The RN will assist the ANFHT in achieving its vision and mission. The role of the RN in the ANFHT requires a thorough knowledge of primary care nursing and the ability to function effectively in a team environment with Physicians, other Inter Health professionals (IHP) and administrative team members. The role also requires a high level of interpersonal skills to deal with patients and their families in a holistic compassionate fashion.
Education and Qualifications:
Professional/Education/Experience Required:
BScN in Nursing
Current membership with RNAO and evidence of liability insurance
Member in good standing of the College of Nurses of Ontario
Professional/Education/Experience Preferred:
Strong clinical skills
Excellent oral and written skills
Competency in computer based programs (Microsoft Office - Word and Excel)
Proficient computer skills, effectively uses EMR for documentation
Autonomous
Ability to work effectively with all members of the health team
Ability to work in an interdisciplinary team environment
Ability to prioritize, manage time effectively and be flexible in a very active work environment
Demonstrated experience in Community and/or Primary care
Demonstrated experience in the area of chronic disease management and prevention
CDE preferred but not required
Preference given to those with at least 2 years of Primary Care experience
Excellent interpersonal and communication skills
Roles & Responsibilities:
Assist ANFHT Clinicians with day to day clinical responsibilities
Recording of vital signs
Wound assessment and care
Administering immunizations and injectable medications as required
Documentation of clinical encounters
Liaison with other health agencies (i.e. HCC, Public Health etc.)
Assessment of patient health care services
Triage of clients based on acuity of problems
Observe, collect and record patient data
Utilize the EMR for messaging, documentation etc.
Liaison with patients regarding laboratory, diagnostic results
Sterilization and maintenance of relevant medical equipment
Facilitate the implementation of relevant infection control guidelines
Ordering and proper storage of vaccines
Assist in the transfer of patients to the ER as needed
Update patient Immunization records
Acquiring and processing patient specimens for laboratory analysis as required
Assist Clinicians with gynaecological examinations and other medical procedures as required
Treatment/Management:
Coordination of diagnostic, therapeutic and management plans with the patient, Family Physicians and other Inter Health Professionals
Participate in the development and implementation of all the ANFHT's general and targeted clinical programs, including (but not limited to): Prenatal program, Anticoagulation program, Health prevention/promotion/education programs, Mental Health program and Urgent Care program
Leadership in Chronic Disease Management programs, involvement in Diabetic Management program
Education/Advocacy:
Help patients to identify and use health resources
Involve patients in decisions about their own health
Support patients in learning how to self-manage care
Initiate and support health education activities
Provision of individual, family, telephone advice to patients
Patient instruction for specific diagnostic tests and therapies
Identify community needs and resources
Provide clinical leadership
Referrals/Collaboration
Work collaboratively with the ANFHT Clinicians to enhance patient care, enhance the Physician-patient relationship and augment the effectiveness and efficiency of patient care
Ongoing consultation with Physicians to ensure the best patient outcomes
Work collaboratively with all other team members to optimize patient care
Liaison with bookings and referrals to specialists, community agencies, hospitals diagnostic and therapeutic services, pharmacies etc.

The Registered Nurse will maintain his/her clinical knowledge and skills by participating in a program of continuing professional development as required by the relevant nursing governing bodies. The RN will ensure that he/she has adequate professional malpractice insurance and will submit the appropriate documentation to the Executive Director. The RN will respect and maintain patient confidentiality at all times and sign and the Aurora-Newmarket Family Health Team's confidentiality agreement.
