Job Description

The Belleville Nurse Practitioner-Led Clinic (BNPLC) is seeking a Registered Nurse to join our interdisciplinary health care team. Since 2010, the BNPLC has been a trusted leader in providing accessible, individualized, fully responsive primary health care in the Quinte Region. With more than 3,500 registered patients with a diverse range of needs, we make a difference every day.

The Position



The Registered Nurse (RN) will provide primary health care, health promotion and disease prevention care. Care will be delivered within the clinic and within the community. The RN will provide comprehensive care for clients with chronic disease, collaborating with or consulting the health care team for the provision of evidence-based care. The RN will assist in the establishment of a network with community agencies for referral and supportive care.

MAIN RESPONSIBILITIES AND POSITION OBJECTIVES



The Registered Nurse will practice within the full scope of practice for RNs and in accordance with the standards of practice as outlined in the following documents:

College of Nurses of Ontario Nursing Act (1991) The Regulated Health Professionals Act (1991)
The RN will be a member of RNAO to ensure liability protection and to participate in professional responsibility and leadership.

General Responsibilities:



Provide primary health care to patients in accordance with standards set by governing body, and that is aligned with the overall strategic direction of the BNPLC Provide health promotion and education across the lifespan Promote disease prevention and provide chronic disease management, primary mental health care support and primary palliative care support Enhance access to care Provide assessment, diagnosis and treatment within the RN scope of practice Collaborate and/or consult as appropriate, with members of an interdisciplinary team within the clinic and externally with the community of care Advocate, educate, and consult as needed Participate in committees or community groups where an identified need for advocacy or representation is required Design, deliver and evaluate health care programs for the BNPLC that are aligned with the strategic direction of the BNPLC and coordinated with all members of the health care team; that address current gaps in service, target population or population health Works in collaboration with an Interprofessional team to safely and effectively provide appropriate primary care to clients.

ADDITIONAL RESPONSIBILITIES



Utilize medical directives where care falls outside the RN scope of practice Plan, implement & evaluate health promotion, disease and accident prevention strategies and programs within the community Coordinate care or services for individuals or families Provide culturally sensitive care Provide wound care Record nursing and medical interventions Submit statistics as required by the Ministry of Health/Ontario Health Participate in training sessions as required by the BNPLC Participate in regular team meetings, and other meetings as requested Participates in the development, planning and evaluation of treatment, education, counselling and health promotion/disease prevention activities of the BNPLC. Provide telephone advice and document all advice and care (per CNO standards of practice) in the EMR Organize and participate in the annual influenza program Perform point of care testing (urine, HCG, rapid strep throat) under medical directive. Facilitate group educational sessions and activities using the principles of child, youth, adult or older adult learning. Conduct in-services and workshops for team members, other health professionals and community partners as appropriate. Assists in the maintenance of a clean, orderly work environment

QUALIFICATIONS



EDUCATION / EXPERIENCE:



Degree in Nursing from an accredited university Registration as an RN with the College of Nurses of Ontario Membership with RNAO 3-5 years related or applicable experience in a community or primary health care setting Additional training or credentialing (e.g. Certified Diabetes Educator, Certified Asthma educator, etc.) is considered an asset. Exceptional understanding of current, evidence based and best practice guidelines Excellent communication and organizational skills Proficient in use of internet, Microsoft software, drug database programs Experience with an Electronic Medical Record, an asset Demonstrated ability to work independently and in a team environment Experience in program design, delivery and evaluation an asset Professional liability insurance Current Ontario Driver's License

JOB REQUIREMENTS:



Ability to travel within the community Adhere to Occupational Health and Safety policies and procedures as per applicable legislation Adhere to all policies and procedures of the Belleville NPLC Ability to work flexible work hours (some evenings and weekends will be required) CPR certification Vaccinated against Covid-19 (minimum of two doses) or have valid medical exemption Current Police Vulnerable Sector Check (less than 6 months old)

Physical Demands:



The position requires minimal physical effort and the workday comprises roughly equal amounts of standing and sitting with frequent position changes.
