Collingwood Neighbourhood House (CNH) is a not-for-profit Society serving the Renfrew-Collingwood (RC) neighbourhood of East Vancouver. CNH's mandate is to promote the well-being of the Renfrew-Collingwood community by providing leadership and working collaboratively with individuals, families, agencies and other groups to develop and support inclusive, innovative, sustainable initiatives and services that respond to the community's social, educational, economic, health, cultural and recreational needs.

RISE is a community-based Community Health Centre (CHC) that is part of the Primary Care Network strategy and funded by the Ministry of Health. RISE CHC supports the health and well-being of Renfrew-Collingwood residents who face barriers to achieving health by offering community-based primary care and health promotion services. Populations of focus include Newcomers, LGBTQ2S+, Indigenous, Isolated Seniors, Youth, people experiencing Homelessness, those working in the Sex Trade, and those experiencing Mental Health and Substance Use issues. Services are delivered by a diverse interdisciplinary team, working with other CNH departments and partnering agencies.

JOB OVERVIEW



We are seeking a Registered Nurse (RN) to join our Community Health Centre Team at CNH. The position provides primary care, health promotion and outreach nursing services as part of an interdisciplinary team. There will be opportunities in this role for case management, wound management, administration of medications and immunizations, performing phlebotomy, triaging same day visits, preforming mental health and addiction care, and chronic and infectious disease management.

SKILLS & QUALIFICATIONS



Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).

Two (2) years' recent related experience.

Knowledge of Community Health Nurses Standards of Practice and Guidelines for clinical practitioners.

Current CPR certification.

An egalitarian philosophy and non-judgmental attitude (i.e. gender, culture, race, sexual orientation, language, ability and more).

Previous experience providing culturally safe, trauma-informed care to marginalized populations.

Trained and proficient in conducting nursing assessments and triaging clients. Experienced in implementing interventions for common diagnoses and specific clinical situations, particularly those relevant to vulnerable populations (e.g. chronic disease, infectious disease, chronic pain, addictions, mental health).

Demonstrated ability to perform focused secondary assessments and develop a plan of care relevant to the priority populations, integrating information from a variety of sources.

Demonstrated ability to employ teaching strategies to transfer knowledge to engage clients, families and groups in planning and self-management of care.

Experience and comfort drawing blood.

Demonstrated ability to provide treatment planning, counseling, crisis intervention, and case coordination.

Demonstrated ability to communicate effectively with colleagues, clients and their families orally and in writing for one-on-one and group settings.

Demonstrated ability to provide health teaching in a group setting.

Demonstrated ability to effectively employ conflict resolution, and reconciliation techniques and approaches.

Experience promoting collaboration, capacity building, teamwork, and communication within diverse interdisciplinary teams.

Experience applying strategies or practices to address social determinants of health and health inequity.

Broad knowledge of crisis intervention and counselling skills.

Broad knowledge of mental health issues, substance use and treatment and harm reduction approaches.

Broad knowledge of local community health, social service resources and referral options.

Excellent planning, prioritization and time management skills.

Ability to utilize Electronic Medical Records, word processing, internet and email software.

Ability to provide services through outreach.

Physical ability to perform the duties of the position.

ASSETS



Additional nursing courses, certifications, or experience in triage, immunization, foot care, PAP, addictions care, wound care, contraceptive management and sexually transmitted infections.

Fluency in identified non-official languages based on neighborhood demographics.

Experience working with medical interpreters.

Experience with OSCAR EMR.

JOB DETAILS (Full-Time)



Salary range:

$41.42-$55.91/hour (based on years of experience).

o Extended health, dental benefits and multi-sector pension plan.

o Annual reimbursement for British Columbia College of Nurses and Midwives licence renewal fees and CNPS liability protection fees.

o Paid vacation starting at 4 weeks per year (based on 35 hours/week) plus paid sick days.

Schedule:

35 hours per week, Monday to Friday or Tuesday to Saturday (7 hours per shift).

Location:

RISE Community Health Centre, 5198 Joyce Street, Vancouver, BC, V5R 4H1.

o As well ascommunity settings in Renfrew-Collingwood.

JOB DETAILS (Part-Time)



Salary range:

$41.42-$55.91/hour (based on years of experience).

o Extended health, dental benefits and multi-sector pension plan.

o Annual reimbursement for British Columbia College of Nurses and Midwives license renewal fees and liability protection fees? (pro-rated to FTE).

o Paid vacation starting at 4 weeks per year (pro-rated to FTE), plus paid sick days.

Schedule:

28 hours per week, Tuesday to Friday (7 hours per shift).

Location:

RISE Community Health Centre, 5198 Joyce Street, Vancouver, BC, V5R 4H1.

o As well ascommunity settings in Renfrew-Collingwood.
