Position Summary

:

The Registered Nurse - Clinic and Street Team (RN- CST) will provide nursing services and case management, as needed, for adults 16 years of age and over with complex physical health, mental health and/or substance use health concerns in Hamilton.

Under supervision of the Clinical Nurse Coordinator of the CMHA Primary Health Care Clinic, in collaboration with the Shelter Health Network, the RN-CST will provide and advocate for accessible, coordinated primary care and mental health and addictions support for adults facing barriers to care.

The RN-CST will have a specialized focus on working with people involved in the criminal justice system, with a history of homelessness or precarious housing, and who are without a primary care provider. The RN-CST will provide care and support services to clients in a variety of urban community settings, including encampments.

Qualifications

:

Candidates must possess a degree in nursing, registration with the College of Nurses of Ontario and current practice insurance. Candidates must have a valid driver's license and a reliable vehicle for use in this position. Experience working with people with serious mental illness and substance use disorder is required. Advanced knowledge of wound care/wound care certification is an asset. The ability to offer French-Language services is an asset. Please quote competition CES2125.
