Your Opportunity!
Reporting to the RAI Lead, The RAI Coordinator is responsible for the on-going management, development, evaluation, and staff education of the InterRAI LTCF Program in providing quality resident centered care. This position works in collaboration with the Nursing Rehabilitation and Restorative Care team (NRRC) and other members of the multidisciplinary team.
Key Responsibilities and Skills:
Manages the co-ordination and completion of the resident assessments, tracking forms, Clinical Assessment Protocols (CAPs) and care plans.
Schedules and monitors all information on residents' assessments.
Responsible for education and software training to appropriate staff members on InterRAI LTCF
Reviews CIHI reports with staff.
Participates in quality initiatives resulting from InterRAI LTCF/CIHI indicators.
Coordinates the MOH Relias LTCF certification of all newly hired registered staff.
Closely monitors RUGs/CMI results.
Responsible to ensure staff education, orientation and mentoring to support the transfer of InterRAI LTCF and Quality Improvement knowledge within the Home.
Understands RNAO Best Practice Guidelines (BPG).
Make positive contributions to the home environment that ensure the values of Primacare(TM) Living Solutions are maintained.
Qualifications
Proficient with InterRAI LTCF and computerized documentation systems
MOH Relias InterRAI LTCF certified, if not, must be completed successfully within one year of being hired.
Registered Practical Nurse with current CNO registration.
First Aid and CPR Certifications preferred.
The ability to read, write, and communicate orally in English.
Experience with PointClickCare.
Experience in LTC preferred.
A Vulnerable Sector Screening check to the satisfaction of Primacare(TM) Living Solutions. One that has been completed with the last six (6) months
Proof of full immunization (2 doses) against COVID-19
Annual Flu Vaccine strongly recommended
