RESPONSIBILITIES:

Employee Performance Appraisal Tracking Sheet, Forms and Procedure

  • Manage Employee Performance Appraisal tracking. In collaboration with department managers ensure completion of performance appraisals, implement procedure and timetable to complete appraisals efficiently record.
Health & Safety
  • Lead and implement Dunk & Associates Health & Safety monthly compliance packages. Communicate to department managers assignments and assist to complete as required.
  • Ensure Health & Safety on-line training is complete, documented
  • Organize Safety Excellence Program annually
  • Manage JHSC monthly workplace inspections, assign to members and coordinate monthly reporting.
  • Manage H&S Staff board, to ensure compliance with mandatory postings.
  • Co-Chair JHSC meetings, document meeting minutes and post
Dunk & Associates Safety Groups Safety Elements
  • Dunk & Associates Safety Excellence program. Lead the program, complete compliance packages, inform all staff and report to Dunk & Assoc an/Ed/JHSC progress. Create binder for managers to review elements policy and procedures.
Orientation Process and Package
  • Complete and organize staff on-boarding for Seaforth LTC and RC. Implement orientation package for Staff and Agency Staff to cover the mandatory education and on the job health & Safety information. Ensure all on-line education has been completed, employee filed complete
Job Schedules and Description
  • Implement a standard format for Seaforth Departmental Job Schedules to include a job description, safe work procedures, hazard Assessments, physical demands analysis.
Fire Drill Scenarios
  • Create written fire drill scenarios for mock drills. Implement a "fire" for sounded alarms (pylon with light on top)
  • Involve JHSC and management on Emergency Preparedness Drills and awareness. Participate in the education and completion of the emergency drills annually.
Emergency Preparedness
  • Lead monthly Quality Meetings, involving an interdisciplinary team to review quality initiative and plans of actions to meet targets. Communicate status to all staff. Review Extendicare Meeting and committee policies and compare the committees established at Seaforth
ARCHES, ALC Program
  • Manager ARCHES, ALC Program with Home and Community Care. 10 bed contract admit/discharge. Manage staffing, job performance. Family and community contact for the program. Arrange ADL support, PT/OT etc. Lead bi-weekly resident rounds virtually. Ensure all RHRA requirements met, care plans, falls, behaviours and medication management processes.
Jazz HR - Human Resources
  • Lead the Jazz HR program for Seaforth LTC and RC. Post, receive and screen all applicants to fill vacant positions to endeavor for a full staff complement. Arrange interviews with department managers, participate in interview and reference. Contact candidate with offer, and requirements before reporting to work, following MOHLTC, MOL and CA guidelines.
QIP
  • Quality Lead. Devices QIP annually, collaborates with stakeholders, implements plans of action and reports status
Accreditation
  • Lead for Accreditation Survey, distribute assignments and responsible for collecting and submitting evidence. Communicate to stakeholders.
Compliance
  • Assist to support home to ensure MOHLTC compliance standards are met. Action plan initiated and completed.
QUALIFICATIONS:
  • Post-secondary education in a related field such as health sciences or adult education.
  • Extensive experience in staff education in a healthcare or long-term care setting.
  • Considerable experience in quality improvement and risk management.
  • Strong behavioural and or dementia supported program knowledge preferred
  • Leadership style that supports a learning environment which encourages participation and builds commitment to organizational objectives and values the contributions of staff.
  • Excellent ability to motivate, lead, train and manage staff in a fast-paced environment.
  • Highly developed interpersonal, conflict resolution, project management and problem solving skills.
  • Ability to interact and communicate effectively both orally and in writing with all levels of the organization.
  • Well developed management level report writing and presentation skills.
  • Proven ability to analyze data, implement quality improvement initiatives, solve problems and resolve conflicts. Sound knowledge of all government legislation related to long-term care and occupational health and safety.
  • Knowledge of public policy, accreditation standards and processes that affect long term care.
  • Proven ability to meet deadlines and manage multiple assignments.
  • Lean/Quality Improvement training at a minimum of Yellow belt level or equivalent Must be flexible with working hours.
