The Salvation Army Toronto Grace Health Centre (TGHC) is a mission of The Salvation Army that provides specialized and integrated care and services to individuals with multifaceted chronic or persistent illnesses that require Complex Continuing Care, Post Acute Care Rehabilitation, Palliative care and Transitional care services. At Toronto Grace we support the patients and families through their recovery and rehabilitation journey following the principles of Patient and Family Centred Care facilitating patient flow through the health care system and the successful reintegration to their communities.



The Quality and Patient Safety Specialist plays a key role in fostering a culture of safety, learning, and continuous improvement across the organization. Guided by the organization's mission, vision, and values, the Specialist supports patient safety through expert consultation, system analysis, event review, and education. This role ensures that safety practices, investigations, and improvement initiatives are aligned with organizational goals, accreditation standards, and best practices in healthcare safety.


Position Summary:




Reporting to the Manager, Quality, Risk and Patient Experience, the Quality and Patient Safety Specialist will be responsible for the design and implementation of key quality and patient safety initiatives across the sits of Toronto Grace Health Centre (TGHC). The Quality and Patient Safety Specialist will work with all units and departments, including front-line staff, managers and directors, and provide assistance and input to the Senior Leadership Team.


The successful candidate will collaborate with all levels to foster an environment of minimizing risk and maximizing patient safety and quality care across the organization, to ensure continued organization wide success. In addition, collaborate with key stakeholders to develop and implement key High Reliability Organization (HRO) tactics and initiatives.


Responsibilities:




Patient Safey Specialist:



Review/audit/edit/monitor patient safety incident reports to ensures accurate documentation, investigation and follow up. Identify trends and emerging risks through data analysis and communicate findings to relevant stakeholders. Lead or support investigations of serious safety events, critical incidents, and quality of care reviews, including liaising with patients and families. Prepare reports and presentations summarizing findings, recommendations, and system improvements. Collaborate with departments and leaders to assess and mitigate patient safety risks in both clinical and non-clinical settings. Working with Enterprise Analytics, Strategy and Transformation, Risk Management, Professional Practice etc. Develop policies, procedures and reports pertaining to Quality and Safety Collaborate with key stakeholders to develop and implement key High Reliability Organization (HRO) tactics and initiatives

Quality Improvement and Program Support:



Support the use of safety and quality methodologies such as Root Cause Analysis (RCA), Failure Modes and Effects Analysis (FMEA), and Lean process improvement. Promote and sustain a Just Culture framework to encourage open reporting and learning from adverse events and near misses. Contribute to the development and implementation of the hospital's Patient Safety Program, Enterprise Risk Management (ERM) framework, and annual Quality Improvement Plan (QIP). Provide input to content of annual Quality Improvement Plan, support the Accreditation process and corporate indicators.

Education and Capacity Building:



Develop and deliver education sessions, training modules, and resources related to patient safety, risk mitigation, and quality improvement in collaboration with the Interprofessional Practice and Education Team. Provide expert consultation and advice to leaders, physicians, and staff on patient safety issues and best practices. Support the onboarding of new staff regarding patient safety principles and reporting expectations. Plan, develop, implement and monitor the Patient Safety Program across TGHC. Collaborate with teams to design and execute safety campaigns and awareness initiatives such as Canadian Patient Safety Week.

Qualifications and Education:




Master's degree in a health-related discipline an asset Must have an undergraduate degree in a health-related discipline from a recognized institution Must be a regulated health professional with certification or experience in a health care discipline/healthcare setting Experience as a system and incident management administrator an asset Experience maintaining and updating policies, procedures and standard work documents. Lean methodology training and monitoring QI initiatives preferred Knowledge of High Reliability Organization (HRO) tactics and initiatives an asset Minimum (5) years' experience in a healthcare setting Minimum (3) years' experience in patient safety, risk management, Incident investigation and analysis, and project management Experience and knowledge with Patient Safety and Quality projects or initiatives, electronic clinical incident management systems, performance measurement, data reporting analysis, quality improvement principles and Quality of Care Reviews Proficient knowledge of legislation and regulatory environment applicable to hospitals/ healthcare, including Excellent Care for All Act, Quality of Care Information Protection Act and Privacy Act

Key Competencies:




Advanced computer knowledge and skills in applications such as, but not limited to, Microsoft Office 2016 (Excel, Word, PowerPoint and Access) and Adobe Acrobat, Visio and Project) Excellent use of healthcare-related e-software {e.g. Incident reporting system, medical records) Sound judgment and critical thinking in complex or sensitive situations. Demonstrated ability to influence and engage stakeholders to promote system-level change. Commitment to continuous learning and advancement of safety culture. Ability to manage multiple priorities in a fast-paced environment.
