Job Overview

We are seeking a compassionate and dedicated Health Care Aide to join our team. In this role, you will provide essential support to patients in various settings, including assisted living facilities, nursing homes, and home health environments. Your primary focus will be on enhancing the quality of life for individuals by assisting with daily activities and ensuring their comfort and safety.

Responsibilities

Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and meal preparation. Monitor and document vital signs and changes in patient conditions. Provide companionship and emotional support to patients, fostering a positive environment. Administer medications as prescribed and ensure proper documentation in compliance with HIPAA regulations. Utilize EMR or EHR systems for accurate medical documentation and record-keeping. Assist with cleaning tasks, laundry, and meal preparation to maintain a safe and hygienic living space. Support patients with mobility challenges using equipment such as Hoyer lifts when necessary. Engage in memory care activities for patients with Alzheimer's or dementia to promote cognitive function. Collaborate with healthcare professionals to develop and implement individualized care plans.

Skills

Strong patient observation skills to identify changes in health status. Experience in assisted living, nursing homes, or long-term care settings is preferred. Knowledge of medical terminology and familiarity with CPR procedures. Ability to provide catheter care and support hospice care needs effectively. Experience working with individuals with developmental disabilities or special needs is an asset. Proficiency in cleaning tasks, cooking, and general caregiving responsibilities. Understanding of medication administration protocols and vital sign monitoring techniques. Commitment to maintaining patient confidentiality and adhering to HIPAA standards.
