Public Health Nurse, Chronic Disease and Injury Prevention

Status: (2) Contract Full Time \xe2\x80\x93 Until December 31st, 2024

Salary Range: $41.49 - $49.24 per hour

Work mode: Hybrid* (Travel required for meetings/resource pick up + community visit) see below for more details about this work mode.

Hours of work: Monday to Friday 8:30AM - 4:30PM (Flexibility is required to attend morning & evening meetings at times)

The School Health Program requires a Public Health Nurse who can build and maintain strong relationships to deliver population health strategies to improve health outcomes for a group of schools across Peel Region.

The individual must work in a self-directed manner within a collaborative team environment to promote and deliver services using a comprehensive school health approach.

Team Specific Responsibilities:

  • Establish and maintain relationships with internal and external partners to identify and meet the needs of Peel\xe2\x80\x99s school-aged children
  • Take the lead role to work comprehensively with school communities on evidence-based initiatives
  • Use theories and principles of health promotion to achieve program outcomes
  • Use health promotion strategies to create supportive environments for healthy eating, physical activity, and mental well-being
  • Support and provide consultation and guidance to stakeholders
  • Experience using Teams, Avaya, Sharepoint
  • Candidate will be required to travel to various school sites across the Region of Peel
What you will do\xe2\x80\xa6
  • Apply knowledge regarding concepts of population health status, inequities in health, determinants of health, and strategies for health protection and promotion to guide practice
  • Utilize research and evidence and apply critical appraisal skills to inform policies, programs and practice
  • Provide health information, teaching and counselling to clients
  • Use a holistic approach to assess the health status and functional competence of clients with consideration of the impact of the determinants of health
  • Identify and analyze how clients\' culture, attitudes, beliefs and feelings about health influence interventions
  • Establish mutually determined goals with client, including outcome indicators and evaluative measures
  • Facilitate client engagement in identifying and defining health issues using community development and community assessment skills
  • Participate in the planning, developing, implementation and evaluation of community wide and site-specific public health strategies using Evidence Informed Decision Making (EIDM)
  • Advocate for and contribute to the development of healthy public policy
  • Collaborate with partners to identify and address public health issues and service gaps in order to achieve improved health outcomes
  • Develop, foster and facilitate inter-sectoral partnerships and networks using team building, negotiation, conflict management and group facilitation
  • Demonstrate culturally sensitive approaches and strategies with diverse populations
  • Communicate effectively with internal and external stakeholders
  • Promote awareness of the Region of Peel and Public Health services
  • Participate in research, quality assurance initiatives, and mentor-ship opportunities as appropriate
  • Adhere to public health and nursing ethical standards and principles
  • Fulfill legal and professional obligations and responsibilities in accordance with relevant legislation and departmental policies, procedures and medical directives/acts as an ambassador for the Region of Peel, Public Health Services and Public Health Nursing
  • Perform other duties as assigned which support the organizational goals
  • Respond to public health emergencies, significant events and incidents as required by the Health Protection and Promotion Act and Emergency Standards
What we require\xe2\x80\xa6
  • A Baccalaureate Degree in Nursing or equivalent recognized by a Canadian University
  • Current license to practice as a Registered Nurse by the College of Nurses of Ontario
  • Experience in public health or community health nursing is preferred
  • Effective written and oral communication skills
  • Ability to work effectively both independently and as part of a team
  • Ability to establish and maintain collaborative working relationships
  • Analytical and problem-solving skills
  • Advocacy, negotiation and conflict management skills
  • Assessment, planning, implementation, and evaluation skills
  • Relevant computer skills (MS Outlook, Word, PowerPoint and Excel) and the ability to work with databases
  • Effective public relations, presentation, public speaking, group facilitation and diplomacy skills
  • Effective organizational and time management skills
  • Ability to apply appropriate theory and learning principles to practice
  • Knowledge of Peel\xe2\x80\x99s diverse community and associated health impacts
Work Mode & Job Location: at 7120 Hurontario Street, Mississauga, along with the ability to work remotely. *Please note: You will be required to travel to various sites in the Region of Peel (Mississauga, Brampton & Caledon).
