The Public Health Nurse is responsible for the provision of nursing interventions that promote and preserve the health of communities, families, and individuals across the lifespan. The Public Health Nurse functions as part of a multi-disciplinary team to plan, develop, implement, and evaluate programs and services provided by the Agency. The Public Health Nurse exercises independent judgment, strong critical thinking skills, and decision-making in the context of the position while prioritizing and expediting service delivery.



This is a temporary full-time position in the Clinical Services program. This position could also be assigned to any of the following programs: Client and Community Services, Vaccine Preventable Diseases, Children's Health Services, Infectious Diseases, or Infection Control based on program needs. Depending on program assignment, the Public Health Nurse will be required to provide on-call, after-hours services.



The Public Health Nurse will be involved in the provision of Community Health/Health Protection programs to clients and community partners across Wellington-Dufferin-Guelph, including:

Support and education to community members, clients, and partners regarding all topics of public health concern; TB Skin Tests and TB active surveillance; Sexual health supports; Harm reduction and needle exchange programs; Case management and contact tracing for diseases of Public Health Significance; Record assessment and immunization provision across a variety of settings; Vaccine storage and handling; Home visiting program for prenatal to early childhood; Mandatory Blood Testing Act and Designated Officer Program; Inspections, education, and training related to Infection Prevention and Control in health care facilities, personal service settings, and childcare settings.

Hours of Work:



Regular office hours are Monday to Friday, 8:30 am to 4:30 pm. May include evening or weekend work. Flexible working hours are required, as per the Collective Bargaining Agreement. Depending on assignment, will be required to provide on-call, after-hours services.

Requirements:



4-year undergraduate degree; Registration as a Registered Nurse with the College of Nurses of Ontario; 1 to 2 years practical work experience in nursing; Demonstrates core competencies for public health practice; Knowledge and experience in case and contact follow-up and public health management of infectious diseases and diseases of public health significance; Knowledge of the Ontario Public Health Standards, including relevant Protocols and Guidelines; Experience working with at-risk and priority populations, and providing education/support in both 1:1 and group settings; Strong skills in client and community assessment and analysis; Experience in policy and program planning, implementation, and evaluation; Value diversity and inclusiveness; Strong communication skills, both verbal and written; Demonstrated leadership and collaboration skills; Demonstrated ability to engage community partners and implement health promotion initiatives and/or program changes; Knowledge and experience/aptitude for computer systems and various software programs including: Outlook and Microsoft Office Suite; Ministry databases such as Panorama, iPHIS, and HCD-ISCIS considered an asset; and Electronic Medical Records (TELUS Collaborative Health Record preferred).


In accordance with the Police Vulnerable Sector Check Policy and Procedure CA.52.01.127 and CA.52.02.127, this position is required to provide and/or obtain a recent satisfactory Police Vulnerable Sector Check (PVSC) at time of hire.
