Reporting to the Manager of Programs and Services, the Dementia Resource Consultant serves as the initial point of contact for hospital staff and potential clients of the Alzheimer Society Huron Perth (ASHP) (persons living with dementia and/or care partners) in the Emergency Department of Stratford General Hospital. The Dementia Resource Consultant works in the Emergency Department of the Stratford General Hospital as part of the Integrated Dementia Resource team (DREAM Team). The Dementia Resource Consultant will provide support connecting persons living with dementia and/or care partners to program and services within the Alzheimer society (such as, Behavioural Support Outreach, First Link Supports, Social Recreation and Respite) and other community resources. The Dementia Resource Consultant facilitates and/or follows their journey through service coordination and collaboration with other staff of ASHP and community partner organizations. The Dementia Resource Consultant educates/models for the emergency department team members behavioural strategies to mitigate the risk or necessity for restraints in the ED using Montessori activities and Gentle Persuasive Approach (GPA) strategies.
Qualifications:
A university degree in social sciences or a related field, or comparable education, along with knowledge and understanding of community resources and services is required.
Membership in a Professional College is preferred.
Demonstrated experience performing assessment, screening and triaging is required.
Demonstrated experience working with persons living with dementia is required along with an in-depth knowledge of Alzheimer's disease and related dementia's including behavioural strategies and GPA techniques.
Valid driver's license and access to reliable vehicle

Specific Job Responsibilities:
Client service experience in healthcare sector, social services sector and community support sector preferred
Experience working directly with people living with dementia and their care partners
Exceptional interpersonal skills including shared decision-making and facilitation
Ability to take initiative, prioritize workload and manage competing tasks
Excellent communication (verbal and written)
Demonstrated ability to work independently and within a team
Expertise and experience in cultural sensitivity and diversity
