About the Opportunity

Reporting to the Manager, Inpatient and Residential Services - Western Zone, and working in residential and day program settings, the Psychosocial Rehabilitation Worker provides holistic care for individuals with severe and persistent mental illness to achieve success and satisfaction in accordance with their personal values and goals. The Psychosocial Rehabilitation Worker supports, educates and assists clients in the areas of living, working, learning and socialization, and facilitates an environment that fosters client participation and independence.

The Psychosocial Rehabilitation Worker participates as an active member of an interdisciplinary team and practices in accordance with the Psychosocial Rehabilitation model, demonstrating a commitment to the mission, vision, goals and objectives of the Nova Scotia Health (NSH). Conducts responsibilities in accordance with NSH policy and procedures, NSH Code of Conduct and Provincial Mental Health Standards

About You

We would love to hear from you if you have the following:

  • Diploma in a Human Services Program or equivalent.
  • One (1) year experience in a health or social services or related field
  • Mentors staff in facilitation skills
  • Determines actions/responses related to emergent client situations
  • Meets with clients to process emergent situation
  • Collaborates with team and contributes to assessment of client needs
  • Supports and assists clients to identify skills, knowledge and resources needed to achieve goals
  • Identifies behaviors that may represent risk to self or others and reports to immediate supervisor for consultation
  • Identifies when a client has an episode of acute illness and, in collaboration with the multidisciplinary team, develops and implements a plan of action.
  • Assists clients to maintain existing support networks and establish new supports as required
  • Establishes and maintains rapport with the client\'s family and/or support network as deemed appropriate.
  • May be required to conduct home and community visits
  • Supports clients in their social activities including decision making, community awareness and community assistance
  • Observes clients and ensures self-administration of oral/topical medications as prescribed and in compliance with guidelines/policy.
  • Monitors and communicates pertinent information, such as ability to manage illness, changes in clients conditions or behaviors to team lead and other team members
  • Monitors and evaluates clients\' needs and strength
  • Monitors the effects of medications on client
  • Monitors clients\' progress and identifies indicators of clients\' preparedness for discharge from the program
  • Communicates and consults with team, psychiatrist, management of external resources to ensure coordination and continuity in client care
  • Maintains and promotes health and safety standards and procedures in accordance with Occupational Health and Safety, Workplace Safety and Patient Safety Standards
  • Works under the direction and supervision of RN/LPN/OT/SW or other discipline with a professional licensing body.

Compensation and Incentives

$23.98 - $26.78 Hourly
