Responsibilities
Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
Monitor patient health by checking vital signs such as blood pressure, pulse, temperature, and respiration regularly.
Administer medications accurately following prescribed care plans while adhering to HIPAA regulations to maintain confidentiality.
Support patients with memory care needs, including dementia and Alzheimer's care, ensuring a safe and comforting environment.
Provide companionship and emotional support to reduce feelings of loneliness or isolation.
Prepare nutritious meals aligned with dietary restrictions and assist with meal planning and cooking as needed.
Maintain cleanliness of living areas through regular cleaning tasks to promote hygiene and comfort.
Use specialized equipment such as Hoyer lifts or resident's personal assistive devices to ensure safe transfers and mobility assistance.
Observe and document patient conditions meticulously, reporting any changes or concerns promptly to healthcare professionals or supervisors.
Follow all safety protocols including first aid procedures, CPR certification requirements, and emergency response plans.
Support residents' participation in social activities or community outings to foster engagement and independence.
Assist with documentation related to care plans, medication administration records, and incident reports in compliance with regulatory standards.
Qualifications
Proven experience in home health care, assisted living facilities, nursing homes, group homes, or long-term care environments.
Knowledge of patient observation techniques and ability to monitor vital signs accurately.
Familiarity with dementia care, Alzheimer's care, autism support, hospice care, and working with individuals with developmental disabilities or disabilities in general.
Certification in CPR and first aid is required; additional certifications such as HIPAA training are preferred.
Experience operating mobility aids like Hoyer lifts or resident's personal assistive devices is advantageous.
Ability to lift heavy objects safely and perform physically demanding tasks regularly.
Strong communication skills for effective interaction with patients, families, and healthcare teams.
