Your Opportunity:
The Program Facilitator - NDD Care Coordinator, located at the Alberta Children's Hospital and Child Development Centre, is focused on supporting collaborative care across tertiary and community-based services for children and youth with Neurodevelopmental Disorders (NDD) and complex care needs. This position is part of the Integrated Brain Health Initiative and supports care integration across hospital-based subspecialties, community-based providers and mental health services. This role works from a team approach, and in continuous partnership with families and service providers to promote: timely access to needed care, comprehensive and continuity of care, and enhancement of child and family wellbeing.
The role includes:
Assessing the patient and family's complex care needs and linking family with appropriate service teams.
Facilitating inter-professional care planning, co-management, and case review.
Leading process improvements to facilitate timely communication, inquiry, and follow-up across disciplines.
Supporting successful transition of care between health services and sectors.
Coordinating access to specialized mental health and subspecialty services aimed at addressing challenges related to self-regulation, communication, and complex behaviours.
Creating vital linkages with community agencies to facilitate access to child and family resources and programing.
Empowering families through coaching and mentorship to build self-managements skills, resiliency and navigational supports to help manage the ongoing care needs of their child. Description:
As a Program Facilitator, you will be responsible for the coordination of services, in addition to operational and clinical leadership. You will support the team to ensure that the services offered are coordinated, effective, evidence based and patient and family centered. Leadership on department/program/service committees is expected. You may also provide varying degrees of direct clinical care at an advanced level. You will provide direction for program evaluation and development within the service or program, and ensure that systems are in place to promote ease of transition between programs internally and externally. You will provide clinical supervision and education to interdisciplinary staff and students in order to support their ongoing day to day professional practice.

Completion of master's degree in Social Work, Psychology, Nursing, Occupational Therapy, Education, Physiotherapy, or Speech-Language Pathology. Active or eligible for registration with the applicable college. Additional Required Qualifications:
Extensive work experience related to pediatric complex care is required:
Three years relevant experience working in a pediatric acute care setting or in community based pediatrics or primary care, particularly in the care and service of vulnerable populations such as children/youth with special health care needs, complex mental health and/or neurodevelopmental disorders.
Demonstrated ability to develop strong partnerships and engage diverse stakeholders in program planning.
Applied knowledge and understanding of families and children with complex care needs and demonstrated ability to identify needs of clients and families in collaboration with other care providers, in order to determine most appropriate service response and accordingly prioritize/triage access to services.
Demonstrated understanding and application of core philosophy or values consistent with a family-centred approach to care.
Demonstrated success working in a multi-disciplinary team.
Understanding of the community services and other government systems (Calgary Zone) that support children with NDDs and complex health needs and their families.
Culturally effective capabilities demonstrating a sensitivity and responsiveness to varying cultural characteristics and beliefs. Preferred Qualifications:
Demonstrated skill in leadership, advocacy, and communication.
Experience with advocacy and community development strategies and implementation.
Familiarity with project management, QI and research:
Able to define scope.
Gather data.
Define targets and goals.
Identify deliverables.
Plan trajectory.
Execute and provide direction for evaluation.
