TNO - The Neighbourhood Organization (TNO) is a well-established community-based social and health service agency providing a wide range of programs and services supporting low-income and newcomer communities across Ontario and sometimes across different provinces through online services and referrals. We are seeking one

Program and Outreach Facilitator

to join our team in a

full-time permanent

role.

The ideal candidate will hold a

diploma or degree in Social Services, Community Development, or a related field

, with

at least two years of experience working with vulnerable clients, preferably seniors

.

Demonstrated experience in recreational programming, program planning, facilitation, and outreach

is essential. Candidates should have strong interpersonal, communication, organizational, and problem-solving skills, with a proven

ability to work independently

and as part of a team.

Experience working with at-risk or frail seniors

, including those with cognitive impairments, mental health, or mild to moderate medical issues, is highly preferred. Knowledge of therapeutic program planning, dementia programming, and community service settings is an asset. Candidates must possess excellent computer skills (Word, Excel, databases, PowerPoint or Publisher), and certifications such as Food Handler's Course, CPR, and First Aid are considered assets. The role requires

flexibility to lead weekend, early morning, and occasional evening shifts

and access to

a reliable vehicle

are required.

DUTIES & RESPONSIBILITIES



Independently plan, set up, and deliver recreational activities with minimal supervision. Designs group session programs and activities promoting seniors' program. Develops monthly work plans and activities. Plans and conducts assigned programs, initiates new programs, and plans special events. Develops partnerships to enhance effective programming. Set Up and Clean Up before and after each program. Lead and facilitate programs for seniors, ensuring they are engaging, safe, and accessible. Conducts orientation session for potential clients prior to registration and intake for the program ensuring eligibility. Plans, organizes, and facilitates group workshops and orientation sessions. Prepares and evaluates appropriate material for group facilitation and workshops in collaboration with other teams and programs within the agency. Coordinates activities with other TNO departments and services. Represents TNO on committees, various sector networks and roundtables through attending and hosting meetings. Facilitate networking sessions. Actively participates in program's community development activities including advocacy, outreach and community engagement and empowerment activities. Includes community development principles in program planning, leads community development activities and ensures allocation of program resources to support community development activities. Participates in networks of neighbourhood based service providers. Nurtures community members for leadership. Participates in and supports local community events. Provides advocacy and acts as a liaison with other service providers. Ensures client's physical safety and emotional well-being as well as all Social Determinants of Health. Ensures all clients and community feedback and concerns are addressed in a timely manner and recorded according to TNO's policies and procedures. Initiates and maintains therapeutic relationships with program participants, their families, and other care providers. Ensures compliance with all health and safety policies. Inputs data into appropriate databases, maintains required program records, and submits required weekly and monthly reports. Coordinates activities with other TNO departments and services. Works with the Seniors Program team to achieve successful outcomes. Actively participates as a team member and works with others to achieve common goals.

Screening

- As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.

Job Type:

Full-time permanent position.

Work Location(s):

45 Overlea Blvd. Toronto, ON M4H-1C3, 10 Gateway Blvd, Toronto, ON, M3C 3A1 and other locations base on program needs.

Schedule:

35 hours per week, including two Saturdays per month, early morning shifts, and some evening hours as required.
