Responsibilities
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
Monitor patients' health status by observing vital signs such as blood pressure, pulse, respiration, and temperature; report any changes to healthcare professionals promptly.
Provide medication reminders and assist with medication administration following HIPAA guidelines and care plans.
Support clients with meal preparation and feeding, ensuring dietary needs and preferences are met safely.
Maintain detailed documentation of care provided, patient observations, and any incidents in compliance with privacy regulations such as HIPAA.
Use specialized equipment like Hoyer lifts or resident lifts to assist with safe transfers and mobility.
Engage clients in social activities and provide companionship to promote mental stimulation and emotional well-being.
Perform light housekeeping duties including cleaning client living areas to ensure a hygienic environment.
Collaborate with healthcare teams to develop, implement, and update individualized care plans tailored to each client's needs.
Assist with transportation for appointments or community outings when necessary.
Ensure safety protocols are followed at all times, including proper use of assistive devices and adherence to infection control procedures.
Support clients with specialized needs such as dementia or Alzheimer's care, memory care, autism support, or hospice care as required.
Respond effectively during emergencies by administering first aid or CPR when necessary and contacting emergency services promptly.
Skills
Proven experience in home health care, assisted living facilities, nursing homes, or group homes is highly preferred.
Strong knowledge of patient monitoring techniques and vital signs measurement.
Familiarity with care for individuals with developmental disabilities, dementia (including Alzheimer's), autism spectrum disorder, or other special needs populations.
Certification in CPR and First Aid is required; additional certifications such as HIPAA training or hospice care are advantageous.
Ability to lift heavy objects safely using equipment like Hoyer lifts; physical stamina for long shifts involving standing or walking.
Excellent communication skills for working effectively with clients, families, and healthcare teams.
Compassionate attitude combined with patience and understanding when working with vulnerable populations.
Detail-oriented with strong organizational skills for maintaining accurate documentation and following care plans diligently.
Knowledge of infection control practices and safety standards within home health or long-term care settings.
