Position Overview:

We are seeking a dedicated and organized

Private Home Care Coordinator

with strong experience in home care. The ideal candidate will oversee client care plans, assist with the care matching and coordination of caregivers, ensure quality service delivery, and actively contribute to the growth of our services through community outreach and marketing initiatives.

Key Responsibilities:

Assist with client intake, assessments, and service agreements. Coordinate and manage individualized care plans for clients. Communicate with clients, families, and caregivers to ensure consistent, high-quality care. Schedule caregivers and maintain accurate staffing records. Monitor client satisfaction and address any concerns promptly. Develop and implement local marketing initiatives to attract new clients. Build and maintain relationships with referral sources, community organizations, and healthcare professionals. Ensure compliance with company policies and industry regulations.

Qualifications:

Health care professional (RN/RPN/LPN/Social Worker/Occupational Therapist/Physiotherapist). Previous experience in home care coordination and case management of the elderly. Strong understanding of senior care needs and caregiver management. Knowledge of marketing and community outreach strategies. Excellent organizational, communication, and problem-solving skills. Proficiency with scheduling systems and basic computer applications. Compassionate, professional, and client-centered approach.
