The PHC Coordinator is responsible for the planning, ongoing development and implementation of the Mobile Clinic as well as other programs, services and initiatives assigned through leadership in each respective portfolio.The scope of practice for the PHC Coordinator will includes collaborative work with health care providers and family physician\'s offices to support change management strategies that are inherent in the services plan for primary health care.
