Visit us today and check out to see why more people from across the globe are moving here.About the OpportunityReporting to the PHC Manager, Zone and/or Provincial and/or the PHC Director, the PHC-DFP Coordinator will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management, and Department of Family Practice (DFP) portfolios. The PHC-DFP Coordinator will have responsibility for coordination of the planning, ongoing development and implementation of programs, services and initiatives assigned through leadership in each respective portfolio.The scope of practice for the PHC Coordinator will include collaborative work with family physician\'s offices and other health care providers to support change management strategies that are inherent in the services plan for primary health care. They are responsible for supporting the work of Primary Health Care and the Department of Family Practice through practice facilitation and practice support program coordination. They will work closely with Family Practices to facilitate and assist with the measurement and evaluation of practice improvements and to share/promote the successes of these improvements internal and external to NSH.About YouWe would love to hear from you if you have the following:

  • Baccalaureate degree preferably from a health discipline/health related science
  • Current registration with professional association if applicable
  • Two years\' of recent related experience
  • Coordination and Project Management Skills
  • Broad knowledge of the health system with high level understanding of population health approaches, community development, primary health care, health prevention and promotion, and disease management.
  • Experience in Practice Facilitation including coaching, facilitation, change management strategies, and quality improvement is an asset.
  • Experience working in a family practice setting with family physicians an asset.
  • Organizational and process management skills. Superior time management skills in order to prioritize and complete assignments.
  • Sound knowledge and skill working in an interdisciplinary settings.
  • Ability to analyze information to identify priorities and emerging issues
  • Superior communication skills - written, oral, presentation.
  • Demonstrated high degree of independent decision making, self-direction, motivation and team building/leadership skills.
  • Understanding of Behavior Change Techniques
  • Competencies in other languages an asset, French preferred
