We are looking for

Team Coordinator

to join our team at CUPS.

The Team Coordinator functions within a Team Based Care environment to provide front-line coordination and support for a multidisciplinary team. The Team Coordinator plays a key role in ensuring clients accessing CUPS receive holistic care from the right provider at the right time. Reporting to the Manager of Clinical Operations, the Team Coordinator is responsible for co-creating a caring, dignified and trauma informed experience for CUPS clients. In addition to providing administrative clinical support, client navigational support, and data management within the health clinic. Team Coordinators direct and maintain patient flow for a team of providers, nurses, and social workers. Team Coordinators will work closely with their immediate team members, quality improvement initiatives, staff throughout the organization, and external stakeholders to support CUPS clients to ensure an integrated approach to service delivery.

STARTING

HOURLY SALARY RATES:

$64,335 to $67,732

Key Responsibilities:



Coordinate the flow of clients and client information through the health clinics multi-disciplinary teams and to internal CUPS programming for achievement of integrated care experience for client: including triage, facilitating assessments, referrals and appointment scheduling Provide timely facilitation of client requests ensuring that care is provided by the right provider at the right time Liaise with members of the multidisciplinary team to support daily scheduling and workload management Champion quality improvement initiatives to support clinic best practices Collect and manage client records ensuring accuracy and completeness, following up with team members as required to acquire missing information Remain up to date on internal changes to programs, services and personnel to ensure accurate care plan development and warm hand-offs of clients to colleagues

Leadership



Create approachable, caring and trauma informed environment for clients accessing CUPS services Contribute to safety and wellness of clients, colleagues and visitors by remaining up to date on CUPS health & safety protocols Provide day-to-day guidance for a multidisciplinary team to provide medical and social supports in a primary care setting Navigate daily challenges for the multidisciplinary team, providing consistency and clarity in decisions and approach Demonstrate a commitment to best practice and evidence-based medical care through participation in quality improvement work Lead daily huddles to support closed-loop communication and timely delivery of services Works collaboratively by creating rapport with team members within the department and across the organization. Act as steward for client dignity by ensuring confidential client information is handled with the utmost care and respect in alignment with HIPA and CUPS privacy protocols & values Act as ambassador of CUPS purpose, ambition and values both internally and externally

Collaborator Relations



Develop and maintain collaborative and accountable working relations with internal CUPS colleagues to strengthen communication flow and cross-program understanding Support effective team-based care through clear communication with colleagues and rotating physicians, including bringing in other team members supporting client as needed (internally and externally) Provide caring, reliable, trauma informed care to all clients accessing CUPS to reduce barriers to service and increase community trust Act as the key point of contact for patients/clients paneled to specific team and coordinate services to meet needs Share experience and knowledge with students, volunteers and guests interested in CUPS services to further develop CUPS social justice impact & external ambassador network

What You Bring



3-5 years working with vulnerable populations, with at least 2 years working specifically in a primary care environment Demonstrated experience providing first point of contact service to diverse variety of clients who may be experiencing mental health, addiction, poverty & trauma concerns Experience and demonstrated effectiveness in navigating group challenges in teams with diverse viewpoints to achieve positive outcomes High skill in relationship development and sustainment with clients, colleagues and external partners Previous experience coordinating the work of others effectively within multidisciplinary team. MOA certificate/diploma or LPN licensure considered an asset. Other health-related certifications may be considered in combination with work experience.

Able to balance numerous demands and conflicting priorities, and strong problem-solving skills. Able to manage multiple priorities effectively; maintaining structure and efficiency in a dynamic work environment. Ability to work co-operatively within a multidisciplinary team with demonstrable skill at navigating change, adjusting priorities swiftly, and maintaining a high level of performance in evolving circumstances Capable of analyzing challenges, making sound decisions and implementing solutions under pressure. Ability to demonstrate patience and understanding for those with addictions and behavioral disorders. Strong computer proficiency, experience with Telus Collaborative Health Record (CHR) EMR an asset Excellent verbal and written communication skills. Must be able to operate with a trauma-informed approach. Understanding and ability to apply principles of Harm Reduction.

Working Conditions



Technology use, including CHR Electronic Medical Records System, Office 365. Periods of stress due to work volume and work pace. May be exposed to infectious waste, diseases, and stressful situations. Manual dexterity required for computer use and medical procedures. Physical activity includes walking, standing, lifting (min 35lb), and patient support. Exposure to trauma and emotionally complex cases.
