POSITION SUMMARY



Reporting to the Clinical Nurse Lead, the Casual Primary Care RN works in accordance with vision and values of North Island Health Society and relevant provincial legislation and nursing standards set out by BCCNP (BC College of Nursing Professionals). Responsible for providing casual non-emergent primary and preventative care services to children, youth, adults and Elders to ten North Island Nations in the communities and in urban settings. (Kwakiutl First Nation, Mamalilikulla First Nation, Dzawad'enuw First Nation, wiwsut'inuw Hxwa'mis First Nation, Gwawaenuk Tribe, Gwa'sala-'Nakwaxda'xw Nations, T'?at?asik'wala'la First Nation, Quatsino First Nation, and Da'naxda'xw First Nation).

Travel and the use of a personal vehicle are requirements of this position.

Principal Duties and Responsibilities



1. Provides holistic patient assessments that include health history, medication history, and physical evaluations with patience and Cultural Humility.

2. Demonstrates cultural humility, trauma and violence informed practice and provides care that acknowledges the client's values, cultural beliefs, and practices. Respecfully acknowledges lived experiences, history and provides nonjudgemental advice/answers to client's primary concerns

4. Monitors, assesses and documents observations, symptoms, interventions and changes in client health status.

5. Where relevant within scope of practice, orders diagnostic testing and/or collects speciimens to make evidence informed decisions for care.

6. Applies health promotion, chronic disease management, advanced care planning, and/or self-care management principles to conduct assessments, identify issues, provide direct care, and address concerns or variances from the plan of care with empathy and Cultural Humility.

7. Administers medications, injections and treatments according to established policies and protocols and within the scope of practice.

8. Collaborates with clients and families to plan, implement, coordinate, and evaluate client care plans in consultation with members of interprofessional health team as needed.

9. Reviews clinical findings with client/family and the interprofessional team, collaboratively identifies client-centred goals and plans for care that promote self-management Primaand improved health-related quality of life.

10. Champions cultural safety and humility in all aspects of the job, and advocates for and applies culturally relevant and appropriate approaches when building relationships and providing nursing services.

11. Maintains competency that reflects current standards of practice, including anti-racism and cultural humility standards as they are developed, by engaging in professional development activities including review of relevant literature, attending educational workshops and in-services, consulting and networking with other health care professionals.

Demonstrates strong understanding of scope of practice and actively seeks out and utilizes appropriate consultation and/or referral to higher level of care when required. Utilize relevant medical history platforms such as Cerner, Medinet, Excelleris in assessment and diagnosis. Demonstrates clear, concise Subjective, Objective, Assessment and Plan (SOAP) charting in community EMR/charts. Perform Phlebotomy and transport specimens to lab in accordance with TDG (Transportation of Dangerous Goods) policies and procedures. Provide referrals and access to community services, allied health care and regional health programming. Demonstrates effective collaboration and communication with existing health care providers and health programmers in community (i.e., Community Health Nurse (CHN), Community Health Representative (CHR), Counselling, Social Worker (SW), Family Support, etc). Collaborates with larger FNPCI health team to meet the health needs of individuals and communities they serve. Maintain detailed and organized patient charts and ensure relevant information sharing is provided to patient care providers when needed. Perform weekly checks of equipment, supplies and medication outdates. Provides monthly Primary Care reports to Clinic Nurse Lead. Various other General Duties

Job qualifications/employment requirements



Qualifications



Bachelor's Degree in Nursing from a recognized university.

Certified Practice preferred (Remote, CM, STI, OAT).

STI and Contraceptive Management and pelvic exam certification an asset.

Minimum 1-year Primary Care Experience or minimum 3 years in related nursing field (ie. Home and community care, community health nursing, public health, etc.).

Valid CPR-C.

Emergency, public health and mental health and addictions nursing experience an asset.

Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).

Requires a valid Class 5 BC Driver's License.

Requires a vulnerable sector check.

Experience



Experience working with First Nations, preferably within the region.

Experience with case coordination and/or case management involving Indigenous communities in a culturally safe manner.

Experience in promoting best practices in Indigenous health.

Experience working in a fast-paced and evolving environment with a concentration in health.

Experience in a provincial regional health authority system/service and in particular working with First Nations individuals, communities or organizations on issues related to health and wellness.

Experience coordinating patient care for a wide range of health issues.

Experience in the mapping of health and/or social systems and assets.

Experience in the development of individual health and wellness plans, particularly for those living with complex health care conditions.

Knowledge



Knowledge and understanding of Kwakwk'wakw history and culture. Knowledge and demonstration of cultural humility and trauma-informed care in all areas of practice. Knowledge of First Nations Health staff, programs and available resources. Knowledge of regional health programs and resources and FNHA nursing practice support. Knowledge of, and ability to apply, an understanding of local First Nations cultural principles and protocols in work situations. Knowledge and the application of concepts, principles, and theories of trans-cultural nursing, including knowledge of traditional, homeopathic or other healing practices used in the community. Knowledge of nursing which includes the nursing process, conceptual frameworks of nursing, theories and principles of nursing practice. Problem solving techniques/skills.
Effective communication skills; team-based practice and strong interprofessional relationship building.
