We are currently looking to hire:

Primary Care Access Navigator - Registered Practical Nurse


2 - Full-Time, 6 Months Contract (35 hours per week)

Purpose:



The Primary Care Access Navigator will play a pivotal role in enhancing access to primary care and health services for unattached individuals and equity-deserving populations. These roles will work directly with the Scarborough Ontario Health Team (OHT) to advance this strategic priority by curating service information, coordinating navigation initiatives, and collaborating with health service and physician partners to implement patient pathways.

The Primary Care Access Navigator will work closely with the Ontario Health at Home Care Connector to follow evidence-based pathways, identify local primary care provider capacity available to immediately attach patients to primary care and to navigate to interim local health and social services. They will also work collaboratively with primary care providers and interprofessional care teams across multiple service providers.

Key areas of responsibility:



Navigation & Implementation

? Lead the development and execution of system navigation strategies aligned with OHT deliverables.

? Curate and maintain up-to-date information on local health services, with a focus on primary care provider access.

? Collaborate with Health811 and member organizations to ensure visibility and usability of navigation resources.

? Access and review information on local primary care provider availability and on local unattached patients, particularly those unattached patients on the Health Care Connect wait list to support connecting patients to a primary care provider.

? Triage registration forms from individuals seeking primary care providers to support matching them to a provider.

Digital Navigation & Analytics

? Assess and implement digital navigation tools that complement provincial platforms.

? Monitor and report on user engagement across OHT digital assets (e.g., website visits, Health811 tile usage).

Community Engagement & Collaboration

? Partner with local providers, community organizations, and Care Connectors to ensure coordinated navigation efforts.

? Support warm transfers and proactive outreach to reduce service drop-offs and improve patient experience.

? Facilitate communication strategies to promote priority services through appropriate channels (web, social media, etc.).

Performance Measurement and Reporting

? Distribute and analyze standardized patient and provider experience surveys focused on navigation.

? Track and report on navigation-related metrics as outlined in the OHT Performance Framework.

? Contribute to monthly, quarterly and year-end reports demonstrating progress on navigation deliverables.

Educational and/or Professional Qualifications:



Current registration with the College of Nurses of Ontario (CNO) in good standing as a Registered Practical Nursing (RPN).

Skills and Attributes:



? Experience in health system navigation, care coordination, and patient engagement within a healthcare or social service setting.

? Strong understanding of interprofessional primary care teams models in Ontario.

? Working knowledge of OceanMD and electronic medical record systems (e.g., PSS, EPIC)

? Comfortable navigating digital health tools and service directories (e.g., Health811, PHSD).

? Demonstrated ability to work collaboratively across sectors

? Demonstrated experience in health equity, social determinants of health and working with diverse populations.

? Demonstrated ability to work collaboratively within an interprofessional care team.

? Experience with the development and implementation of effective education programs for patients and families.

? Proven ability to effectively and efficiently coordinate, implement and monitor a patient's plan of care/service.

? Excellent communication skills with proven ability to collaborate with patients, caregivers, and members of the health care team in various settings.

? Demonstrated telephone assessment and triage skills.

? High degree of professionalism and initiative, with the ability to function independently with limited immediate supervision.

? Proven good attendance with the capability of maintaining the same standard.

? Experience working with equity-deserving communities.

? Experience working and delivering health and social care services in Scarborough.

? Familiarity with Ontario Health's Care Connector model and Health Care Connect program.

Remuneration:

$27.70 - $32.93 per hour
