Job Description

As an integral part of the

Recovery Care

team, the Registered Practical Nurse will work within their scope of practice and authorized medical directives to deliver patient-centered care in multiple environments including a mobile clinic, and/or inpatient stabilization, and treatment unit in Ottawa West.

General Responsibilities:



Open and close the work area including exam rooms daily. Review and action any phone messages. Startup/shut down computer systems and other hardware and assess functionality ongoing. Greet and process patients in a professional and organized manner. Always ensure excellent patient flow throughout the clinic. Verify health insurance information and accuracy of patient information and follow through on any requirements related to health insurance validity. Manage all messages from physicians, pharmacies etc. Prioritize, and complete daily Process mail, scan documents, faxes, general data entry, filing. Support physician and advanced practice providers for requirements related to virtual care services. Assist in training and development of new staff. Maintain exceptional documentation throughout all position responsibilities. Always connect back to the chart to update actions/encounters. Evening shifts and weekends

Clinical Responsibilities:



Complete and process urine drug screening (UDS) as required and dispose daily as needed to assist the Medical Office Assistant during periods of high patient volumes. Complete electrocardiograms as required. Complete new patient assessments as prep for the physicians and advanced practice providers, ensuring all necessary paperwork is completed. Complete new patient assessments on all patients who are new to the clinic as well as patients who have not been seen in ?3 months. Evaluate all patients for barriers to program access and help to reduce barriers to increase retention and engagement. Complete assessments for all patients including questionnaires related to substance use as applicable (i.e. COWS for Suboxone). Complete assessments for all patients. Carry out appropriate nursing interventions as required, including but not limited to:
o Medication and vaccine administration.
o Vital signs
o Wound assessment and management
o Electrocardiograms (ECGs)
o Substance Use and Harm Reduction education
o Laboratory functions (phlebotomy, centrifuge, specimen collection, specimen processing)

Process all specimens in a timely manner as per corporate, public health and ministry requirements. Receive, review and scan all laboratory and general nursing documents. Prepare/disinfect exam rooms for patients and procedures and ensure final check/clean of exam rooms at the end of each day. Ensure all required medical equipment is in working order and report to management Completes all charting as required by licensing and facility standards. Record all procedures as required and complete billing daily. Respond to patient telephone inquiries and follow up with patients regarding test results/changes and provide teaching on topics as required. Maintains inventory lists and physical levels of medical and laboratory supplies daily/weekly and stock exam rooms at the end of each day. Practice using appropriate infection control precautions at all times. Assist in the training and development of all new staff. Monitors vaccine levels, completes required logs and orders vaccines as equired to maintain appropriate levels of inventory. Ensure compliance with Fridge Vaccine Guidelines including log sheets. Reports and handles any/all cold chain breaks as required by Public Health and Recovery Care Management. Complete all assessments and documentation for related nursing programs as per Recovery Care policy and nursing standards. Troubleshoot patient barriers to program and health resource access and problem solve to improve patient outcomes and increase retention in programs. Review tracking sheets for the HCV treatment program and follow up with patients that have not returned for follow up of their HCV infections to ensure they are following through the cascade of care. Development and provision of patient care resources. Identify gaps in programs and services from patient feedback and work with management to address the gaps. Other duties as required, fitting with scope of practices

Qualifications:



Valid registration as an RPN in good standing with the College of Nurses of Ontario. Diploma from an accredited nursing program. BLS or must obtain within 6 months of hire.
