Multimorbidity, or the presence of two or more non-communicable chronic diseases in an individual, may be one of the greatest challenges facing health care. The Canadian Collaboration for Complex Care (C4) is developing personalized health solutions by focusing on the promotion of situational literacy. We define situational literacy as an enhanced understanding of the interaction between the person, the complexities of their lived experience and the complexities of their clinical conditions. We propose that this understanding is fundamentally important to developing effective personalized care plans for patients with multimorbidity. Our overarching goal is to demonstrate a transformative and transportable approach to designing tools and environments that promote personalized decision-making.
